                                      FOR WEEKLY TIMESHEET

          STANDARD WORK WEEK BEGINS SATURDAY AT 12:01 AM AND ENDS MIDNIGHT FRIDAY

Employee:

    1.    Print your name.
    2.    Sign your name after your furlough work week is recorded in the spaces provided.
     3.   Post work week ending date for the furlough work week.
     4.   Post dates of work week.
     5.   The row labeled Flex Hrs. before SAT and after FRI may be used for flex schedule hours
          coming from the previous work week or going to the following work week.
     6.   Post the IN and OUT times of each work day (please include AM or PM).
     7.   Post LUNCH time ONLY if you take a lunch. If you do not take a lunch,
          then run a line through Lunch/Break(s).
     8.   Record any paid leave taken by writing the type of paid leave under the Paid Leave Type
          column and record the hours for the paid leave under the daily hours column.
    9.    Record the furlough day by writing FURLOUGH under the Unpaid Leave Type Column
          and record the hours for the unpaid leave under the daily hours column.
          This column can be used for any other type of unpaid leave by recording the type of
          unpaid leave under the Unpaid Leave Type column & the hours under the daily hours column.
   10.    Post total daily hours under 'Daily Hours' for each day.
   11.    Record total hours for the furlough work week.
    12.   Turn Weekly Time Sheet in to Supervisor.


Supervisor:

    1. Verify posted time.
    2. Sign and date timesheet(s).
    3. Deliver to Unit Time Manager so that each furlough weekly time sheet for each employee
       is filed in their employee file at the work unit for auditing purposes.
