LETTER OF AGREEMENT BETWEEN NONPROFIT ORGANIZATION AND MARYLAND
NONPROFITS FOR THE EMPLOYEE ASSISTANCE PROGRAM SERVICES
Maryland Nonprofits agrees to provide the following Employee Assistance Services:
Assessment/diagnostic interviews-each employee family unit is allowed up to three visits during a
twelve month period, at no cost, for assessment and referral.
Twenty-four hour telephone service - the EAP is available 24 hours daily by telephone for questions,
requests for service, and urgent situations.
Referral assistance to participants in accessing outside services and treatment resources. Follow-up
of all referrals and monitoring of continuing care.
Supervisory consultation by telephone to supervisors regarding "troubled employees" and job
A minimum of one supervisory training per year to be held at the Maryland Nonprofits offices.
Policy consultation by telephone regarding the EAP and related issues.
Brochures for each eligible employee.
Maryland Nonprofits guarantees the rate of $2.50 per employee per month through June 30. Notice of any
rate change will be made prior to May 1.
The nonprofit organization agrees to enroll all eligible employees who work a minimum of ______
hours per week in the EAP.
The nonprofit organization agrees that premiums will be paid by the first of the month for the EAP
The nonprofit organization agrees that the minimum period for this service is twelve months
beginning with the first of the month following the signing of this letter of agreement.
The nonprofit organization agrees that any additional services will be billed separately and will be
paid within 30 days of invoice date.
The nonprofit organization may terminate this contract at any time after the initial twelve month
period upon giving 30 days notice.
