							                     UNIFORM RENTAL AGREEMENT


I, ______________________________________________,               will be responsible

for the care and return of the Perrysburg High School Band uniforms. I agree to

pay a replacement fee as stated below if any part of this uniform is not returned to

school as directed to do so by the band director. I also agree to be responsible for

hemming the pant length and will pay any repair costs if any part of the uniform

is damaged in any way.
