CLINICAL EXPERIENCE

General Dentistry
  Westmead Hospital Dental Clinical School (1988-90):

    Rostering through the various specialised Units with involvement in a variety of
    post-graduate clinical and didactic experiences.

    Considerable experience in providing on-call "after-hours" emergency dental care
    in the setting of the Casualty service, Department of Accident and Emergency,
    Westmead Hospital.


  Private Practice Experience (1989-92)

    Over an extended period undertook permanent part-time work in the following
    general dental practice - Dr Ken Lipworth and Associates, Blacktown Pacific
    Medical Centre, 36 Kildare Road, Blacktown NSW 2148.

Co-ordinating Dental Officer.
Primary Oral Care Unit, Department of Assessment and Diagnosis (1989).

Responsible for the assessment and management of patients presenting for
emergency dental care. Other duties included organising and allocating Dental
Officers in undertaking procedures required in the management of such patients.

In addition this position involved care of medically compromised patients -
specifically dealing with Head and Neck Radiation Oncology patients. This
included treatment planning of these patients prior to radiotherapy, follow-up care
and ongoing monitoring of their oral mucosa.

Participation in the Disaster Victim Identification and Forensic Dentistry Course
(1989). Practical experience has been gained in victim identification by ongoing
participation in individual cases and a member of the Dental Team involved in the
Kempsey Bus Crash (December 1989) and the Wallsend Hostel Fire (June 1991) and
Leura Train Crash (2000).

a.     Oral Medicine

          In conjunction with and under the supervision of both Professor D.M Walker
          (Chair of Oral Pathology, Faculty of Dentistry) and Assoc. Professor A.P.
          Barrett (Head of Oral Diagnosis Unit, Member of Department of Medicine,
          Westmead Hospital and N.S.W Dental Board Registered Specialist in Oral
          Medicine) intensive experience has been gained as a Resident and Registrar in
          both the general and specialised areas of Oral Medicine.

       General Oral Medicine

          I have been responsible for conducting the management of the Oral Medicine
          Clinics organised on an out-patient basis within the Dental Clinical School.
          This included the Joint University & Hospital Oral Medicine Clinic run in
          conjunction with Professor Walker which involved the following: accepting
          consultations, undertaking and following up pertinent special investigations,
          initiating care for such new patients, as well as, ongoing review of existing
          patients.

          Additional activities also undertaken include:
          Oral Biopsy Service; and the provision of a
          Clinic for Medically-compromised Patients. Broad experience has been
          gained in conducting a Clinic both for the assessment and the provision of
          general dental care to moderate-to-severely medically compromised patients
          (including HIV-infected and other infectious risk and immuno-compromised
          patients).

          Oro-Facial Pain Clinic. This clinic focuses on the assessment and
          management of patients with chem-sensory and neurological impairments of
          the oral and maxillofacial complex.


     Specialised Oral Medicine

       This involved providing ongoing care, accepting consultations and assessment of
       medically compromised and immuno-suppressed hospital in-patients. In
       particular, experience has been gained in the care and presentation of patients
       with haematological malignancies, patients undergoing intensive chemotherapy
       and bone marrow transplant recipients. This requires daily ward rounds in the
       Clinical Haematology/Oncology Unit, Westmead Hospital as well as
       participation in the Unit's weekly patient care review meetings.


     Internal Medicine

       This has involved extended periods (of 6-8 weeks duration) of active
       participation in the outpatient services and clinics, as well as limited involvement
       in the in-patient activities, in the following medical disciplines: Dermatology,
       Oto-laryngology, Neurology, Immunology and Radiation Oncology.

b. Dento-Maxillo-Facial Radiology

     Supervised experience has been gained in general oral and maxillo-facial
     radiography (Mr B. Waters M.I.R) and in diagnostic radiographic work-up and
     interpretation (with Dr A.P. Barrett and Dr M.I. Coombs, Senior Lecturer, Oral
     Diagnosis and Radiology, Faculty of Dentistry, University of Sydney). This also
     involves review of cases referred to the Unit for dento maxillo-facial radiography
     and providing consultations within the dental school. Subsequently I have
     received Honorary appointments with the University to provide teaching at both
     the graduate and undergraduate level in dento-maxillofacial radiography and
     interpretive diagnostic radiology.


c. Oral Pathology

     This has involved weekly attendance of both oral and general histopathology slide
     sessions conducted by the Consultant staff of the Department of Anatomical
     Pathology, Institute of Clinical Pathology and Medical Research, Westmead
     Hospital, as well as provision of relevant clinical and radiographic interpretation
     (and diagnosis) of pertinent cases.


(Acting) Head of Oral Diagnosis
     The focus of clinical activity since 1995 has been the oral diagnostic sciences.
     This has included personal patient care, research, clinical training of dental
     undergraduates and post-gradates, and where relevant, clinical supervision of
     medical post-graduates, all within the context of a major medical/dental tertiary
     referral teaching institution.

     The principal areas of involvement have been in the areas of clinical Oral
     Medicine, and the management of significantly medically compromised patients.
     There has also been ongoing provision of general consultative services in Dento-
     Maxillo-Facial Radiology.

     Clinical experience has covered the spectrum of routine (and specialist) oral
     medicine and head and neck pathology. A major involvement has been in the
     field of the relationship between oral and internal medicine, with particular
     interest in the dental management of oral manifestations and complications of the
     immuno-suppressed patient, particularly in the area of clinical
     haematology/oncology

     Appointment as the Acting Head of the Oral Diagnosis Unit has involved a
     personal commitment to the ongoing development of a comprehensive clinical
     service in the Oral Diagnostic Sciences, fully integrated with the relevant
     specialist dental and medical units throughout the Hospital.

TEACHING EXPERIENCE
Since 1991 experience has been gained in the professional education of dental
undergraduates and post-graduates, medical graduates, dental chair side assistants,
registered nurses and pharmacists. This has involved the presentation of tutorials to the
Medical and Dental Registrars, Dental post-graduates, Registered Nurses (Clinical
Haematology Unit), Hospital Pharmacists, Dental Officers and Dental Chair-Side
Assistants.

Clinical supervision and training has been provided for both undergraduates and
graduate Dental Officers in the diagnosis and management of patients presenting for
emergency dental care to the Primary Oral Care Unit, as referred patients presenting to
the specialist clinics in Oral Medicine and for the medically compromised.

Honorary Tutor, Department of Pathology, Faculty of Medicine (1994)

      This has involved direct face-to-face tutorial sessions to IIIrd year dental
      undergraduate students in bench-top light microscope histo-pathological diagnosis
      of oral and general pathology.

Honorary Lecturer (Oral Medicine), Department of Oral Pathology and Medicine,
Department of Dental Science, Faculty of Medicine and Dentistry, University of
Melbourne, Melbourne, Victoria (1995)

      Lectured to Vth year dental undergraduate students on the oral manifestations and
      complications of the immuno-suppressed patient (AIDS/ bone marrow transplant
      recipients).

Honorary Tutor (Oral Medicine), Discipline of Oral Pathology and Oral Medicine,
Faculty of Dentistry, University of Sydney (1995 - 2000)

      This involved the undertaking of tutorial sessions with the Vth year dental
      undergraduates in both practical (clinical) oral medicine, as well as provision of
      didactic sessions in oral medicine.

Honorary Tutor (Oral Radiology), Discipline of Oral and Maxillofacial Surgery,
Faculty of Dentistry, University of Sydney (1995- )

      Involving the provision of tutorials to IVth and Vth year dental undergraduate and

      During the course of 1996, under the auspices of Dr Malcolm Coombs (Senior

ADMINISTRATIVE EXPERIENCE

CLINICAL ADMINISTRATION

  This has included involvement in the following:

    Dental Officer (Paediatric Dentistry)
    Responsibility for arranging in-patient admissions and the weekly operating lists
    for patients requiring General Anaesthesia.

    As Co-ordinating Dental Officer (Primary Oral Care Unit)
    Responsible for organising the activities of the Dental Officers and Final Year
    Dental students rostered to the Primary Care Unit.

    Resident/Registrar (Oral Medicine)
    Undertake the organisation of all Clinics. This involves arranging of
    appointments, acceptance and responding to referral letters, instigating
    appropriate special investigations and following up results, arranging further
    referral or consultations as required and clinical audit and updating of the recall
    list.

    (Acting) Head, Oral Medicine Unit
    This has involved the instigation of treatment for all newly referred patients (both
    from the out-patient clinics as well as in-patients) and acting as a consultant in all
    matters pertaining to the discipline of oral medicine.



GENERAL ADMINISTRATIVE EXPERIENCE

  WESTMEAD HOSPITAL

    Within the Dental Clinical School, and in the course of my present position, I
    have been involved in routine department/unit organisation, supervision and
    policy making procedures, implementation of Hospital-wide EEO principles, and
    Dental School recommendations and monitoring of infection control guidelines
    ("Universal Precautions Policy").

    Participation on various committees devoted to: general Dental Clinical School
    management, infection control, treatment of the medically compromised patient,
    drug review, and the provision of specialist services.

    Contributions to the Operational and Policy and Procedures Manual of the
    Westmead Hospital Dental Clinical School, in the form of reviewing and updating
    the manual in readiness for accreditation.

    Participation in Strategic Planning Conference, planning the future directions of
    the Dental School, with specific responsibility for the Oral Diagnosis Unit.

  (Acting) Head, Oral Medicine Unit

    In my capacity as Head of Unit, I undertook a comprehensive review of the
    Department’s activities. Following on from this I instigated structural changes to
    the department’s administrative an clinical organisation, so resulting in a more
    stream-lined service, and removing barriers to patient flow and care. This also
    involved the development of clinic for significantly medically-compromised
    patients. Included in this was a review of chair side and reception staff functions,
    with a move to the development of an integrated, multi-skilled support staff. This
    resulted in reformatting of the department structure, acknowledged with the
    change of the department’s name to that of: Department of Oral Diagnostic
    Sciences (DODS).


  THE WESTMEAD ASSOCIATION

    As part of the Executive of this organisation, which is devoted to the didactic (as
    well as social) interests of the Hospital's clinical (medical and dental) staff.

    Appointment as Honorary Treasurer has involved appropriate budgeting and
    auditing of the Association's affairs (operating budget exceeds $100 000 per
    annum). As a committee Member of the Executive I have been principally
    responsible for the organisation of Hospital's didactic programs, specifically the
    annual Hospital Scientific Week, with special care for the running of the Dental
    Symposium(s). This has involved the organisation of the main presenters,
    chairpersons and speakers, as well arranging, printing and co-ordinating of
    advertising flyers and the official Program (previous symposiums: Head and Neck
    Malignancy, The New Genetics and Dentistry, Oral-Facial Pain - Diagnosis and
    Management, and Amalgam - The Risks versus the Reality).

    In 1995-96 as Co-Convenor, I shared in the responsibility of organising and
    conducting the whole of the 1996 Westmead Hospital Scientific Week. This
    week represents a period of intense didactic and social activities, involving the
    whole of Westmead Hospital.



PAST ADMINISTRATIVE ACTIVITIES

     Whilst Co-ordinating Dental Officer (Primary Oral Care) -
    I was responsible together with members from other involved Units
    (Restorative/General Dentistry and Endodontics) in establishing an integrated
    protocol for dealing with patients presenting with dento-alveolar trauma to the
    Unit.

    Honorary Secretary, Westmead Hospital Dental Officers Association (1989) and
    as a Member of the Post-graduate Committee (1990-94) for this organisation with
    a shared responsibility for conducting post-graduate Seminars for Dental Officers,
    Residents and Registrars in training.

    Committee Member (alternate Dental Clinical School representative) of the
    Infection Control Committee, Westmead Hospital. (1991-92).

    Committee Member of the Organising Committee for the Third Annual
    Scientific Meeting of the Australasian Academy of Dento-Maxillo-Facial

     the summer vacation period in the Department of Pathology, University of

     1997. Awarded the Iain McKenna Annual Travelling Scholarship awarded by
     the Westmead Hospital Dental Officers Association for research conducted into
     the malignant potential of oral lichen planus and the relationship between
     dentistry and infective endocarditis.
