Gifts and Bequests by Will Form
The Australian Bureau of Statistics says that one in five Australians will suffer from mental illness at some point in their lives. The WA Association for Mental Health (WAAMH) is the peak mental health representative body, for non-Government mental health organisations in Western Australia. Please consider naming The WA Association for Mental Health Inc. (WAAMH) as a beneficiary in your Will so that we may continue to improve the situation for people affected by Mental illness in Western Australia. We would be more than happy to discuss this with you and provide any information you may require. The following Form of Bequest is the easiest method to use to include WAAMH in your Will.
I give and bequeath to the WA Association for Mental Health Inc. the sum of $………………………….…. and/ or the following property: …………………………………………………………………………………………, free from all State Death Duty and Federal Estate Duty to be used for the general purposes of the WA Association for Mental Health, or (state the specific purpose, or intention): ………………………………………………………………………………………… ………………………………………………………………………………………… ………………………………………………………………………………………… ………………………………………………………………………………………….. I direct that the receipt in writing of the WA Association for Mental Health shall be sufficient discharge to my trustees for the payment of such bequest.
