   Why Did You Choose An Independent School?


This survey is aimed at identifying why parents choose to send their child/children to
Independent schools.

All information provided by parents will remain confidential.

You should complete the survey ONLY if you have a child currently enrolled in an
Independent school in either:

     ∗    a Preschool/Preparatory Class;
     ∗    the first primary grade offered in the school (usually Year 1);
     ∗    the first secondary school grade offered in the school (usually Year 8); or
     ∗    has transferred from another school at the end of 2005 or during 2006
          (and is currently in Years 2-7 or 9-12).


All responses should relate only to this child, and the reasons you chose
an Independent school for this child.

If you have more than one child who meets the above criteria, and you have received
more than one survey, please complete a survey for EACH child.



Please place your completed survey form in the envelope provided and return it to your
child's Independent school by 21 August 2006. The school will return all envelopes to
ISQ. Alternately, if you wish, the survey can be returned directly to ISQ.
