                  FOR UHG’S MEDICAL EVIDENCE SOLUTIONS

Introduction

The benefits of using UHG for your clients’ health screening requirements include:
   Web requesting of all health screening requirements including mobile pathology testing,
   paramedical and own-GP examinations,
   Improved quality of the medical requirements collected,
   Email notifications of case progress,
   A single point for on-line tracking of all medical evidence requirements.


What does this mean for you now?

•   Advisers should let clients know that UHG may be contacting them to conduct medical
    examinations on behalf of the Life Office. A copy of a flier to provide to your clients is
    available for downloading on UHG Online.
•   GP Examinations have until recently been the responsibility of the Adviser to arrange.
    Whilst this option is still available to you we strongly encourage you to take advantage of
    using UHG to coordinate these examinations.


Your options?

1. The easiest method of arranging requirements is to allow the Life Office to arrange them
   just as they do for any PMARs.

2. Advisers may still arrange pathology and medical examinations for their clients just as
   they do now. The most effective way of doing this is via UHG’s On-line Request Portal at
   https://client.uhg.com.au/lifedata/Agent/Agentlogin.aspx

    To do this you will need:
       o Your adviser number.
       o Client’s name, address, DOB, and contact numbers.

       Once past the ‘Application Details Entry’ screen and into the ‘Request’ screen, select
       the requirements.

3. Alternatively, you can download the relevant request forms from the front page of this
   site and fax them to UHG on 1800 707 697.


Multiple medical requirements, including Personal Medical Attendant Reports
(PMAR)

Please note that PMARs are always arranged by the Life Office.

New Features

The new site requires you to log in. This improves security and allows the site to pre-
populate your details.

If you currently have a subscription to the UHG Lifetrack Online website at
http://www.uhg.com.au/lifetrack, then your username and password for that site can be
used to login here as well. If not then you will need to subscribe. You can do that from the
login screen.

Apart from the improved security and ease of navigation the enhanced site provides new
features intended to make administration easier for you.

Your username no longer has to be your email address.

You will see that the subscription page (shown below) provides for two email addresses. If
you enter an administrative email address then all notifications about case progress will go to
that address. If left blank then the personal email address will be used. Emails not related
to cases will continue to be sent to your personal email address. Multiple advisors in a group
can use a common administrative email address.

There is also a Group Admin ID on the subscription page. This allows a single user to view
the case progress for all members of a group. To allow this the UHG ID of the administrative
user should be added in the Group Admin ID of all group members. The UHG ID of a user
can be found by selecting the Update/Manage details link on the Welcome page.




How to login to UHG using UHG Online?

Go to web address: https://client.uhg.com.au/lifedata/Agent/Agentlogin.aspx

Save this in favourites for future reference.

You will see a login screen.




You will be taken to the screen below if you need to subscribe.

To register advisor codes for life offices use the section at the bottom of the screen. Select a
life office from the dropdown list, enter your code in the box opposite and click Add. You can
repeat this procedure to add codes for multiple life offices.

How to lodge new requirements with UHG using UHG Online?


When you log in you will see a Welcome screen.

From here you can:

1. Organise a new request online
2. Be redirected to LifeTrack
   OnLine to view detailed
   information of status of all
   evidence requirements managed
   by UHG that are linked to your
   Adviser Number (instructions
   detailed later)
3. Print out a request form for
   faxing new requests to UHG
4. Print out a client information
   leaflet to provide your clients
   preparing them for further
   evidence requirements
5. Information as to benefits of
   using UHG
6. Review or print further copies of
   this instruction manual.

                                                            If click on “arrange a test” you
                                                            will be taken to the screen
                                                            opposite.

                                                            Your details will be prepopulated
                                                            according to the records on our
                                                            database. If you have registered
                                                            an advisor code with us this will
                                                            be added when the appropriate
                                                            life office is selected.. PLEASE
                                                            DO NOT OVERWRITE any of the
                                                            details that are pre-populated
                                                            since this will interfere with your
                                                            ability to monitor cases on UHG
                                                            LifeTrack. If details are incorrect
                                                            please click on the Change
                                                            details link and make the
                                                            changes there.

The change details page is virtually identical to the subscribe page shown above.

NB: If you find that features do not work you may need to modify your security settings; see
‘INSTRUCTIONS IF YOU FIND FEATURES OF UHG ONLINE NOT WORKING’ at end of this
document.

The Applicant last & first name, date of birth, gender, address and policy number are
required fields. At least one applicant phone number is required.

When you have completed entering data on this screen, press Continue. If any required data
has not been entered you will immediately be advised of this.

A request page will be launched and you
simply tick the specific requirements. If
you tick Fasting MBA 20 this will
automatically tick Fasting Lipids to avoid
possible confusion by pathology providers.

To request tests or procedures not listed
select them from the dropdown box under
‘Other procedures/services’. .

Any comments or special instructions
regarding the procedures or preferred
appointment times & etc may be entered
in the Notes/Comments/Instructions box.

When complete press Continue.

                                                                  In all cases a confirmation screen
                                                                  will appear.

                                                                  This screen allows you to check
                                                                  that the data entered is correct
                                                                  before it is submitted.

                                                                  If you discover any errors use
                                                                  the browser’s back button to go
                                                                  to the appropriate data entry
                                                                  page and correct the errors.

                                                                  NB. The data is not submitted
                                                                  until you have clicked the
                                                                  Confirm button on this page.

                                                                  Do NOT click the confirm button
                                                                  more than once.

If you discover a mistake after you have clicked the confirm button email the correct
information to UHG: adviser.relations@uhg.com.au or call 1800 101 984.

You will receive a message telling you that the request was received successfully or that an
error occurred.


On-line progress tracking of medical requirements arranged with UHG

The real-time status of all medical requirements arranged with UHG, including UHG Pathology
requests and PMARs requested via the Life Office that are linked to your Adviser number, can
also be viewed through UHG Online.

Browse to https://client.uhg.com.au/lifedata/Agent/Agentlogin.aspx

Save this in favourites for future reference.

                                                         As mentioned earlier, you will first be
                                                         asked to login. Please see page 2 for
                                                         further details, including how to
                                                         subscribe.

                                                         Once logged in, you will be taken to the
                                                         Welcome Screen pictured left.

                                                         Please click on the ‘view current progress
                                                         of evidence’ section in order to view the
                                                         Lifetrack Online section of UHG Online. It
                                                         is no longer necessary to enter login
                                                         details again.

A summary listing of all cases linked to your login that are pending or either completed or
terminated within the last 30 days will show in the bottom half of the screen. The ‘contacted’
column lists the last contact date by UHG in pursuit of each respective evidence requirement.
Clicking on any specific case will take you to a more detailed summary, including the
progress case notes reading real time from UHG’s tracking data base.

INSTRUCTIONS IF YOU FIND FEATURES OF UHG ONLINE NOT WORKING…

The UHG Online Portal uses javascript for data validation and to enhance user experience.

This means that scripting must be enabled in your browser for these features to work.

If you find that features do not work you may need to modify your security settings. Rather
than lower your general browser security you can add the UHG Online Portal to your list of
trusted sites.

You can do this by following the steps set out below. (This assumes you are using Microsoft
Internet Explorer.)

Open your browser and select the Tools item in the menu bar.




This will bring up the Internet Options dialog box. Select the Security tab and click on the
Trusted sites as shown below.




Click the Sites button and a new dialog will appear entitled Trusted sites. An example is
shown below.

Type https://client.uhg.com.au into the box labelled ‘Add this web site to the zone’. (Note
the ‘s’ at the end of https.) Click the Add button and the address will appear in the ‘window’
beneath as shown below.




Close the dialog boxes by clicking the OK button in each until all have disappeared. The full
features of the site should now be available.
