ACTION: TERMINATIONS-Staff
Attach a copy of the letter of resignation, which indicates the last day of work, and
ensure that vacation and overtime entitlement are noted in the Department comments
section. If vacation pay is not noted, the record of employment (ROE) for
unemployment insurance (UIC) will be wrong. Be aware that the employee is entitled to
have an ROE FIVE (5) days after the last working day.

Faculty Records automatically reinstates a faculty member or librarian when the
sabbatical ends. It is not necessary for the department to complete an EMPLOYEE
TRANSACTION FORM (ETF) if the employee is returning to active status at 100%
salary, (e.g. Exceptions where an ETF should be done would be when the employee
returns from sabbatical and is on a Reduced Load, Leave of Absence or Terminates).

Faculty Records also changes the account subcodes back to 1010 when the sabbatical
ends. If there are account changes other than 1100 returning to 1010, the department
must initiate an EMPLOYEE TRANSACTION FORM (ETF) for the budget change.

Indicate First Time Sabbaticals in DEPARTMENT COMMENTS.

If the sabbatical ends early (e.g. due to illness), and ETF should be forwarded to
Faculty Records.
