      AVPF Summary Performance Evaluation Form

The evaluation form must be reviewed with your supervisor before being presented to
your employee. This will aid in maintaining consistency within your organization and
enhance the overall effectiveness of the performance evaluation process.

There are three major sections to the Summary Performance Evaluation (SPE) form - 20 work
values and behaviors, a set of major areas of responsibility (MARs) that are tailor-made to each
position, and a comments section. The work values and behaviors and the MARs are equally
weighted for this process.

Definitions of the work values and behaviors are listed on pages 2-5 of the Rating Overview
and Guidelines document. Please use these definitions as a guideline to your employee’s
rating. Each value must have only one rating or an error message will appear in the overall
rating area. Each major section will have an average and the four sections combined will
produce an overall average rating for your employee.

Each position will have a listing of the major areas of responsibility (MARs). The weight of
the combined MARs must equal 100% or there will be an error message reflected in the total
area. The employee should be rated as to how he / she accomplishes each MAR based on the
rating categories listed on page 1 of the Rating Overview and Guidelines document.

The score for each of the two sections is then combined to give the overall rating for your
employee. The scale is listed in the bottom right hand section of the form. Note that each
rating has a range.

Comments are required, and should be written to support the rating of your employee.
Comments should cover both the work values and behaviors and MARs.
