                          YOUTH MEMBER REGISTRATION FORM

                                   GUIDELINES FOR LEADERS

Who must complete the Youth Member Registration Form?

A Youth Membership Registration form is to be completed and signed by both
parents or the adult with custody or day to day care of the child. Ideally both parents
should sign the one form however separate forms may be used.

When must the form be signed?

The Youth Membership Registration form must be completed when a girl attends her
first Girl Guide Unit meeting or activity.

Refusal to sign

When the parents or guardian refuse to complete and sign a membership application
form their child cannot be accepted as a member of Girl Guides Tasmania.

Clause 7 of the Youth Member Registration Form, which relates to the use of
photographs, videotapes etc may be deleted by the applicant, if desired.

Requests to delete or change ANY OTHER PART of the form should be referred to
the State Executive Officer of Girl Guides Tasmania.

Youth member photo

Parents or guardians are requested to attach a recent head and shoulders photo of
the youth member to the Youth Member Registration Form however this can be
emailed to Guide House if it is taken with a digital camera.

This photo will be used on the youth member’s membership card and will be kept at
Guide House in case of emergency, eg the youth member is lost during a Girl Guide
activity.

If the parents or guardians do not wish to supply a photo they must contact the State
Executive Officer to discuss their concerns and to make other arrangements.

What to do with the forms?

Completed forms should be forwarded to Guide House, 25 Kirksway Place,
BATTERY POINT TAS 7004 immediately.

Unit records

It is recommended that a copy of the Youth Membership Registration Form be kept
by the Unit Leader. If you do not have access to photocopy facilities, contact your
District Leader or Guide House.

Unit details

It is suggested that Unit Leaders complete the Unit, District and Division details on
the Youth Member Registration Form before handing it to the parent/guardian. This
will ensure that Guide House has the correct details for their database records.

Retention of forms

It is important that Youth Member Registration forms be retained for a period of at
least six years after the member turns 18. Guide House is responsible for keeping
the records for this period.
