intern's supervisor. The information in this appraisal will be kept in the intern's file during his/her career with
SCTC.

Partially Achieves Standards/ 2 performance is below acceptable level and often does not meet
Expectations:                   minimum expectations because of unsatisfactory errors
Unacceptable:                 1 performance is inadequate and fails to meet minimum requirements
