       m a i n t a i n i n g growth, and secondary d i e t a r y d e f i c i e n c i e s may be produced. The

     s c h o o l and c o l l e g e s t u d e n t s , and a l c o h o l induced a c c i d e n t s . P u b l i c awareness
     and s o c i a l concern h a s l e d t o t h e development o f a v a r i e t y o f p r e v e n t i o n
     s t r a t e g i e s , many i n s c h o o l s e t t i n g s . E v a l u a t i o n s o f t h e s e s t r a t e g i e s s u g g e s t
     t h a t school-based i n t e r v e n t i o n s , e s p e c i a l l y t h e more t r a d i t i o n a l d i d a c t i c
     approaches, have n o t been p a r t i c u l a r l y e f f e c t i v e . I n some cases i n c r e a s i n g
     s t u d e n t s ' knowledge h a s even l e d t o i n c r e a s e d u s e o f a l c o h o l . The new
     approaches ( t e a c h i n g p e e r r e f u s a l and s o c i a l s k i l l s , c o r r e c t i n g p e r c e p t i o n s o f
     s o c i a l norms) show some promise b u t a r e n o t uniformly s u c c e s s f u l . A t t i t u d e
     and b e h a v i o r change h a s been more s u b s t a n t i a l i n t h e h i g h e s t - i n t e n s i t y ,
     multi-modal programs, u s i n g p e e r o r p a r e n t l e a d e r s , and " b o o s t e r " s e s s i o n s .
     Though c o s t l y , s o - c a l l e d l f a l t e r n a t i v e s programs" t h a t p r o v i d e h e a l t h y
     o p p o r t u n i t i e s f o r s t i m u l a t i o n , l e a r n i n g , and r e c o g n i t i o n have shown l i m i t e d

     e x p e r i e n c e s , have r e s u l t e d i n a c t u a l changes i n d r i n k i n g and/or d r i n k i n g and
     driving behavior.
     S i n c e t h e p r e v i o u s r e s e a r c h e f f o r t s have y i e l d e d e q u i v o c a l r e s u l t s , t h e r e i s
     much t o l e a r n about what p r e v e n t i o n s t r a t e g i e s might be more e f f e c t i v e . There
     i s a l s o a need t o a d d r e s s t h e methodological and d e s i g n problems i d e n t i f i e d i n
     p r e v i o u s s t u d i e s . A p p l i c a n t s a r e s t r o n g l y encouraged t o i n v o l v e s t a t i s t i c i a n s
     and o t h e r m e t h o d o l o g i s t s i n developing t h e i r p r o p o s a l s .
     SPECIFIC OBJECTIVES
     The o b j e c t i v e o f t h e proposed r e s e a r c h i s t o r e d u c e t h e i n c i d e n c e and
     p r e v a l e n c e o f a l c o h o l - r e l a t e d problems among youth as measured by b e h a v i o r
     change. I n v e s t i g a t o r s may f o c u s on one o r more o f t h e s e problems, e . g . , age
     of o n s e t , frequency o f d r i n k i n g , and a l c o h o l - r e l a t e d a c c i d e n t s . I n d i r e c t
     problems a s s o c i a t e d w i t h a l c o h o l consumption, such a s poor s c h o o l a t t e n d a n c e ,
     may a l s o be c o n s i d e r e d i f j u s t i f i e d on t h e b a s i s o f r e l e v a n c y . I n a d d i t i o n ,
     changes i n c o g n i t i o n , b e l i e f s , o r e x p e c t a t i o n s , and t h e i r r e l a t i o n s h i p s t o
     b e h a v i o r may be measured. If it i s d i f f i c u l t t o measure changes i n a l c o h o l
     abuse d u r i n g t h e time frame o f t h e s t u d y , proxy outcome measures may be
     proposed and j u s t i f i e d .              Some may p r e f e r t o f o c u s on s o - c a l l e d p r e c u r s o r s o f
     l a t e r a l c o h o l problems such a s c h a r a c t e r t r a i t s o r i n d i c a t o r s o f poor s o c i a l
     a d a p t a t i o n ; however, r e l e v a n c e must be proven a s t h e i n v e s t i g a t o r moves away
     from a f o c u s on a l c o h o l problems p e r s e .

While t h e o b j e c t i v e must be a l c o h o l r e l e v a n t , o t h e r h e a l t h i s s u e s such a s
smoking, u s e o f i l l e g a l s u b s t a n c e s , s e x u a l p r a c t i c e s , and/or n u t r i t i o n may b e
a d d r e s s e d . A t h e o r e t i c a l o r pragmatic r a t i o n a l e f o r a d o p t i n g t h e broad
approach must be provided and a s t r o n g a l c o h o l - s p e c i f i c component t h a t i s
r e a d i l y i d e n t i f i a b l e , t r a c e a b l e , and e a s i l y e v a l u a t e d must be i n c l u d e d .
I n v e s t i g a t o r s should be p r e p a r e d f o r t h e p o s s i b i l i t y t h a t t h e i n t e r v e n t i o n may
have u n a n t i c i p a t e d e f f e c t s t h a t could even e x a c e r b a t e a l c o h o l problems.

g u a r a n t e e g e n e r a l i z a t i o n o f t h e r e s u l t s , o r p r o v i d e a clear r a t i o n a l e �or
t h e i r exclusion.
RESEARCH INTERVENTIONS
While o t h e r i d e a s a r e welcome, t h e N I A A A i s e s p e c i a l l y i n t e r e s t e d i n g r a n t
a p p l i c a t i o n s d i r e c t e d a t norm-setting a s a p r e v e n t i v e i n t e r v e n t i o n f o r
c h i l d r e n , a d o l e s c e n t s , and young a d u l t s by: p a r e n t s and f a m i l i e s ;
primary-care p h y s i c i a n s and t h e i r s t a f f s ; youth p e e r groups; elementary and
secondary p u b l i c and p r i v a t e s c h o o l s o r s c h o o l systems; c o l l e g e s and
u n i v e r s i t i e s ; o r o t h e r community o r g a n i z a t i o n s t h a t have d i r e c t c o n t a c t w i t h
c h i l d r e n , a d o l e s c e n t s , o r young a d u l t s . A p p l i c a t i o n s t h a t propose
i n t e r v e n t i o n s t a r g e t e d toward combinations o f t h e s e p o p u l a t i o n s may a l s o be
submitted.
A norm i s d e f i n e d a s an a u t h o r i t a t i v e and b i n d i n g s t a n d a r d f o r a c c e p t a b l e
b e h a v i o r . By communicating a s p e c i f i c s e t o f e x p e c t a t i o n s r e g a r d i n g
a c c e p t a b l e and u n a c c e p t a b l e b e h a v i o r s , norms reduce ambiguity and p r o v i d e
s t r u c t u r e i n s o c i a l s i t u a t i o n s . They can be s t a t e d t o communicate t h e
consequences f o r t r a n s g r e s s i o n s a s w e l l a s rewards f o r a d h e r e n c e . For
example, a s c h o o l may communicate norms t h r o u g h set p o l i c i e s governing t h e
( n o n l u s e of a l c o h o l on t h e s c h o o l grounds and r e s u l t a n t p e n a l t i e s .
The concept of norm-setting i s meant t o convey t h e i d e a o f e s t a b l i s h i n g new
norms o r m a n i p u l a t i n g e x i s t i n g norms through p o l i c y changes or program
i n t e r v e n t i o n s . Other promising p r e v e n t i o n s t r a t e g i e s ( i n t e r v e n t i o n s t o
enhance p e r s o n a l competenc:y) may a l s o be i n c l u d e d w i t h n o r m - s e t t i n g .             However,
t h e independent and i n t e r d e p e n d e n t ( s y n e r g i s t i c ) e f f e c t s o f t h e v a r i o u s
s t r a t e g i e s must be measurable.
Education i n s t i t u t i o n s f o r t h e h e a l t h p r o f e s s i o n s a r e i n an advantageous
p o s i t i o n t o s t u d y t h e u s e f u l n e s s of n o r m - s e t t i n g a s a p r e v e n t i v e i n t e r v e n t i o n .
P h y s i c i a n s and n u r s e s i n primary care m u s t f r e q u e n t l y p r o v i d e i n f o r m a t i o n o r
a s s i s t a n c e on a l c o h o l problems. Under t h e r u b r i c " a n t i c i p a t o r y guidance,"
t h e y may r e n d e r t i m e l y p r e v e n t i v e d i r e c t i o n . Norm-setting may occur w i t h i n
t h e b o u n d a r i e s o f medical p r a c t i c e s and/or w i t h i n t h e l a r g e r town o r community
of t h e physician.
