This Letter of Intent to Relocate is provided by one parent to another whereby one parent gives notice to the other of his or her intention to relocate with the child(ren). Many states require a parent to give written notice of his or her intent to move. This document sets forth the format of what information should be included in the letter of intent to relocate. It provides the relocating parent's new phone number, the name(s) of the child(ren) relocating with the parent, and the reason for relocation. This should be used by a parent when relocating with a child.
