 ‘yes’ Time is included. Free first 20 minutes of calls to other mobiles on the Optus network between 8pm and midnight
 every evening. If call lasts more than 20 minutes and is to a Dodo Postpaid Mobile number, Dodo to Dodo call value
 will be consumed first and if exhausted excess usage will apply to the portion above the 20 minutes.

 #Excess data is charged at current data rates ($2 per MB) and is included in the cap

 CVC is printed on the card - generally this is a 3 or 4 digit number. This number is printed on your Master Card
 & Visa cards in the signature area of the back of the card. (it is commonly the last 3 digits in the signature area
 of the card). You can find your four-digit card verification number on the front of your American Express credit
 card above the credit card number on either the right or the left side of your credit card.

 Declaration and Acknowledgement
 I acknowledge that:
 1. I am authorised to request the porting of the listed mobiles number to Dodo.
 2. I have been advised that by porting the mobile numbers listed on page 3 of this application to Dodo, the mobile
 service and/or any related services associated with that mobile number may or may not be disconnected from my
 current mobile service provider, and may result in finalisation of the account for that service.
 3. I authorise the mobile numbers listed on page 3 of this application, the network type and my request for this number
 to be ported to Dodo, to be disclosed to other service providers to enable the porting of my mobile number, to allow
 call and message routing, for customer and network fault management and for the prevention of fraud.
 4. I acknowledge that I have been advised by Dodo that:
 4a. Although I have the right to port the above mobile number, there may be costs and obligations associated with my
 existing mobile service and the porting of this mobile number.
 4b. I may or may not be in an existing contract with my current mobile service provider, which may or may not include
 an obligation to make early termination payments to that provider.

 Agreement to Terms & Conditions
 By signing and submitting this application you agree to accept the Terms and Conditions under our Dodo Australia
 Business Terms Standard Form of Agreement (“Agreement”). The Agreement can be found on our website at www.
 dodo.com.au/business/terms.aspx

 Please confirm that:
 1. If we consider it relevant to assessing your application for commercial credit, we may conduct a commercial
 credit check and a consumer credit check in relation to your application for credit. You agree to us obtaining from
 a credit reporting agency a credit report containing consumer credit information about you in relation to
 commercial credit provided to you.
 2. You agree to us obtaining personal information about you from other credit providers that may be named in a credit
 report, for the purpose of assessing your application for commercial credit.
 3. You agree that we may obtain a consumer credit report about you from a credit reporting agency for the purposes of
 assessing your application for credit and of collecting overdue payments relating to commercial credit owed by you.

 Additional Conditions:
 1. You agree that the Dodo Fair Go policy applies to the use of all mobile services. Specifically, you agree that any use
 of mobile services for re-sale, re-supply, SIM boxing or other automated dialer equipment is in breach of the policy and
 may result in immediate suspension or termination of services and contract termination fees.

 2. You agree that a minimum contracted term applies for the first 24 months of your Agreement. Should your
 Agreement come to an end during this period (other than through our default), you will be liable for the minimum
 monthly fee for each remaining month of the service, per service,. You will also be liable for any remaining handset or
 other equipment repayments for the remainder of the 24 month period.
