employees enrolled in the UNUM Long Term Disability (LTD) Programs.

employees enrolled in the health insurance plans.

                        **NOTES OF IMPORTANCE**

 Our benefit package adds significantly to the value of
  each covered employee’s total compensation. (30-35%)
 The employee is required to complete their 90 day
  probationary period before they are eligible for the
  majority of our benefits. (Military Leave & the 401(k)
  Plan are exceptions)
 Health insurance benefits become effective the 1st of
  the month following 90 days of full time employment for
  weekly paid employees.
 Health insurance benefits become effective the 1st of
  the month following date of hire for salaried employees.
 Employees who do not elect coverage at this time will
  not be allowed to enroll again until open enrollment in
  November.

                             **NOTES OF IMPORTANCE**

 For FT hourly employees, ADP mails information on benefit
  enrollment 60 days after hire date. All benefit information is now
  accessed online.
 When an employee leaves the company, medical insurance
  benefits will terminate on the day following the last day worked.
  COBRA will be offered.
 Employees on unpaid LOA are responsible for paying their
  monthly group premiums.
 When hired, employees, who are eligible, are automatically
  enrolled in the 401(k) Retirement Plan. They are enrolled at 3%
  deferral, short horizon fund, and their spouse or estate is set up as
  their beneficiary.
 If they would like to opt out of the 401(k) plan, or if they want
  different options, they MUST complete a Change Form and it MUST
  be included in their new hire paperwork!

  and services)
 Call the CIGNA Healthcare 24-Hour Health Information line to speak
  to a registered nurse any hour of the day or night.
