									             DESMOND Programme Annual Report (Summary)

                            (Feb 2008 - March 2009)

Author: Verona Bryant, DESMOND Co-ordinator/Team Leader Primary Care Dietitian
EXECUTIVE SUMMARY
 From Feb– March 2009(14 months), approx 784 referrals were received for DESMOND across the four collaboratives.
 Central are our top referrers (accounting for 38% of referrals), followed by Northe East (22.5%),West Haringey (21%) & South
East (18.5%). West Haringey site moved to Crouch End Health Centre.
 Since Jan 08. 50(88%) GP Practices have referred to DESMOND & only 7 Practices (12%) largely located across Central & North
East Collaboratives have never referred to DESMOND (see those highlighted in red).
 Waiting times are currently up to 19 weeks, this will be reduced when additional programmes are delivered in new year.
 We have invited 776 English clients and have run 35 English DESMOND programmes and 7 Turkish Programmes over the
     14month period.
 A total of 271 participants attended session one & 217 participants attended both sessions one and two. The Turkish participants
account for 40 particpant at session one and 32 at session two.
 On average 35% of those invites attend the programme with an average return rate of 80-81% across both the English and
Turkish programmes. Reasons for not returning for 3/11 participants included sickness (2) & work commitments (1).
 Qualitative feedback from participants (over 100) participants evaluation forms completed):
 Participant reported they were shocked, depressed, tired, very worried, frightened, hopeless, very unsure and unconfident in
themselves since being diagnosed. After attending the programme they felt, less scared, more confident, felt ‘good’, better able to
manage their diabetes. Participant quote “I feel more positive re the future and impact I can have on my health future”.
 The most beneficial aspects of the programme were found to be learning about diabetes with other people newly diagnosed in a
group, the dietary knowledge gained (particularly foods high in fats and sugars and reading labels), and information on physical
     activity.
 Participants expressed increased motivation to make changes to manage their diabetes better by eating a healthy diet and
increase their level of physical activity. Participant Quote: “I never expected to enjoy it so much.”
 Changes participants would make to the programme included nothing on the whole but a few suggested changes were to include
a sample daily menu with info on portion sizes, longer sessions to allow more time to hear other people’s experiences and
     feelings.

 From April 2009 we are increasing the number of programmes delivered from 3 to 4 programmes a month and have extended the
referral criteria to those diagnosed with Type 2 diabetes in the last 3 years. Posters and flyers have been produced to reflect this
       change.
