Submission requirements for Visteon Sterling PPAP Functional Approval: Completed Part Submission Warrant (PSW) signed by Visteon Corp. Supplier Development Eng (SDE) found in Scorecard/Report Card (signature mandatory if submitting Level 2, 3, 4, or 5) Complete description/reason for PPAP submission (drawing change, process change, etc.) Copy of the Visteon "Supplier Process Characteristic Analysis" Copy of the Visteon Approved SCR Two sample parts from production equipment identified as PPAP sample #1 and PPAP sample #2 A drawing with each characteristic/feature/note assigned its own unique number (a.k.a. ballooned dwg) 100% Lay-out of both samples corresponding to each labeled feature and numbered samples Signed and dated copy of your PFMEA and Control Plans Copies of ES test documents (current within one year) Name, phone number, fax number, and E-mail address of supplier contact Samples to be separately boxed and not co-mingled with production and labeled/identified as follows: a) part number b) drawing revision level c) submission level d) PPAP sample #1 and PPAP sample #2 Label boxes using correct Sterling label from PowerPoint file "IQ Labels-060404" dated 060404 Samples and associated paperwork (hardcopy only; no FAX or electronic submissions) submitted to: Sterling Incoming Quality Office Attention: Quality Control Supervisor Ship samples to; Visteon Sterling Plant 39000 Mound Road Sterling Heights, Michigan 48310 Note: hand delivered package is preferred delivery method. Sterling will schedule a Functional Trial Run (FTR) upon receipt of the 300 piece PPAP Functional samples. Functional approval will be awarded to the supplier once the FTR is satisfactorily completed and the appropriate internal signatures are obtained. IQ will FAX the "Functional Check Report" as Sterling's Functional Approval to the supplier. Please note: only Sterling's IQ Supervisor, Sterling's SDE, and/or Sterling's Quality Supervisor are authorized to grant Part Functional Approval. The supplier is then expected to read releases and ship per requirements.
