                             Ontario Student Assistance Program
1)       All students requesting financial assistance for the summer session (May 2010 to August 2010) must complete a
         UWO Full-time Undergraduate Summer 2010 OSAP Application. Please refer to the chart below. If you are
         eligible for OSAP and have not previously applied for OSAP for 2009-2010, you must complete a 2009-2010
         OSAP booklet or web application in additional to this Undergraduate Supplemental Application. The OSAP
         booklet or web application can be found at http://osap.gov.on.ca.

2)       Students enrolling in one half distance studies course or one full distance studies course ONLY must complete a
         “Part-time Canada Student Loan” application. This application is available in Student Central, Western Student
         Services Building. For more information on the “Part-time Canada Student Loan” program, please arrange an
         appointment with a Student Financial Aid Officer by calling 519-661-2100 or visiting Student Central, Western
         Student Services Building.

3)       Please notify Student Financial Aid, in writing, of any changes to your application. Change forms can be
         found in Room 1100 or Student Central, Western Student Services Building or by downloading from:
         http://www.registrar.uwo.ca/FinancialServices/OSAPForms.cfm
         Changes to application can be mailed, dropped off in Room 1100 or Student Central, Western Student Services
         Building.

4)       Ensure a timely application. Release of funding may be impacted by government deadlines.

5)       PLEASE NOTE - False or misleading statements on your application may result in suspension of funding and
         prosecution. All applications are subject to audit.

                          Applications may take up to 6 weeks to process.
