Take notice that you are hereby required to quit and deliver to the owner (Landlord), or his authorized Agent possession of the above described premise now held and occupied by you at the expiration of thirty (30) days after service of this notice. If you fail to quit and deliver possession legal proceedings will be instituted against you to obtain possession and such proceedings could result in a judgement against you which would include costs and necessary disbursements. Please note the rent for the entire period must be paid, even if a earlier departure is made. Re-renting of the premises in the event of an early departure shall obligate owner or authorized agent, to refund a pro-rata portion of the rent paid by you provided all rent and charges were paid. If a tenant with a periodic tenancy other than a tenancy from week to week, is 60 years of age or older or has a physical or mental disability, the tenant may request to be allowed to continue in possession for an additional 30 days beyond the time specified herein by submitting a written request for an extended period and providing proof of his age or disability.
