To be completed by all student participants before departing on a University sponsored field trip.

To be completed by faculty to record all student participants’ emergency contact information before departing on a
University sponsored field trip.



3. Student Authorization to Operate Privately-Owned Vehicle

To be completed by all student participants who will operate a personal vehicle before departing on a University
sponsored field trip.



4. Student Air Travel Voluntary Participation

This notification is used to advise the students traveling by air that they undertake air travel at their own risk.



5. Student Air Travel Informed Consent Statement

To be completed by all student participants traveling by air before departing on a University sponsored field trip.
