AIA Contracts as modified by the Owner
Workers' Compensation Certificate required for Ohio-based firms; or if firm is not Ohio-based, it must provide Workers' Comp Certificate, or mark box on Certificate of Insurance showing Workers' Comp Coverage. If the E & O deductible is greater than $25,000, Architect must supply audited Financial Statements. All design services with a PO only must have the Owner's General Terms & Conditions attached (this will be processed by the Purchasing Dept.) Deductible amount must appear on Certificate of Insurance.

AIA Contracts as modified by the Owner Contractor's who subcontract the hauling of abated material must provide a copy of sub's Certificate of Insurance showing their MCS-90 and other insurance coverage.
Workers' Compensation Certificate required for Ohio-based firms; or if firm is not Ohio-based, it must provide Workers' Comp Certificate, or mark box on Certificate of Insurance showing Workers' Comp Coverage.
