PART I - STATEMENT OF WORK 1. INTRODUCTION / BACKGROUND CH2MHill Plateau Remediation (CHPRC, The Buyer) provides oversight and management as a Prime Contractor to the U.S. Department of Energy (DOE) at the Hanford Site in Washington State. The Buyer is initiating demolition activities on large structures that require heavy demolition equipment to complete. 2. DESCRIPTION OF WORK – GENERAL Buyer requires a Contractor to deliver two medium sized demolition multi-processors and one demolition shear per the following requirements. The contractor shall perform work off site, at the fabricator’s facility. The end result of this contract is to deliver the processor and shear to the 200 Area on the Hanford Site. 3. DESCRIPTION OF WORK – SPECIFIC 3.1. Task 1 Description The Contractor shall supply the processors in accordance with the requirements identified in the “Medium Demolition Processor Specification” (Attachment A). 3.2. Acceptance Criteria for Task 1 3.2.1. Complete working processors. Final working verification of the processor will be performed on the Hanford Site at the service shop in the 200 E Area. 3.2.2. Processors shall pass pre-use inspection by the Buyer in accordance with Quality Assurance criteria listed in Section 6.3. 3.2.3. Processors shall meet all contractual requirements included in this Statement of Work and Specification. 3.3. Task 2 Description 3.3.1. The Contractor shall supply the demolition shear in accordance with the requirements identified in the “Demolition Shear Specification” (Attachment B) 3.4. Acceptance Criteria for Task 2 3.4.1. Complete working demolition shear. Final working verification of the processor will be performed on the Hanford Site at the service shop in the 200 E Area. 3.4.2. Shear shall pass pre-use inspection by the Buyer in accordance with Quality Assurance criteria listed in Section 6.3.

REQUEST FOR PROPOSAL NO: RMM 180668 TITLE: Demolition Processors and Shear 3.4.3. Shear shall meet all contractual requirements included in this Statement of Work and Specification. 3.5. Task 3 Description Upon delivery and acceptance of the processor and shear at the Hanford Site, the Contractor shall provide a total of eight hours of equipment operation instruction to Hanford personnel. Training will be held at a mutually agreed upon date and time in the badged parking lot at the service shop in the 200 E area of the Hanford Site, approximately 30 miles north of Richland, Washington. 3.6. Acceptance Criteria for Task 3 3.6.1. Hanford personnel will be trained in equipment operation as instructed by qualified Contractor personnel. 3.7. Special Requirements 3.5.1 Certified parts and service locations for the processor must be within 150 miles of Richland, Washington.
3.8. Work Location/Delivery 3.8.1. Processor and Shear shall be delivered to the 200 E Area on the Hanford Site, approximately 35 miles north of Richland, Washington. A pre-use inspection will be required at 2355 Stevens Dr., Richland, Washington, prior to delivery to the 200 E Area. 4. TECHNICAL REQUIREMENTS All Work shall be performed in strict accordance with the following requirements, design criteria, national codes and standards, specifications, drawings, exhibits, and any other documents, which by this reference are made a part of the Statement of Work. All inspection of the Work required by governmental agencies shall be arranged by Buyer. 4.1. Codes and Standards 4.1.1. All components shall be designed, procured, tested, and/or inspected in accordance with recognized industry codes or standards. 4.2. Specifications 4.2.1. Attachment A, “Demolition Processor Specification.” 4.2.2. Attachment B, “Demolition Shear Specification”

REQUEST FOR PROPOSAL NO: RMM 180668 TITLE: Demolition Processors and Shear 5. PERSONNEL REQUIREMENTS 5.1. Training and Qualification 5.1.1. Contractor shall ensure that its personnel meet and maintain the appropriate training, qualification and certification requirements. Hanford site-specific general training requirements to safely perform any on site work are identified below: Hanford Site Orientation (HSO) The required training shall be completed prior to work.
5.2. Security and Badging Requirements 5.2.1. For training on site work, see Special Provisions – On Site Services SP-5 for details. 5.2.2. The Contractor shall wear a Buyer-issued security badge identifying himself/herself. A minimum of two working days advance notice is needed for site badging. 5.2.3. Contractor employees will be required to submit to vehicle searches and not personally carry or transport certain prohibited articles. 5.3. Site Access and Work Hours 5.3.1. The Contractor will have access to the training site on the specific dates and times to be coordinated with the Buyer after confirmation of delivery dates. 6. ENVIRONMENTAL, SAFETY, HEALTH, AND QUALITY REQUIREMENTS The Contractor shall perform work safely, in a manner that ensures adequate protection for employees, the public, and the environment, and shall be accountable for the safe performance of work. The Contractor shall comply with, and assist the Buyer in complying with Environmental, Safety, Health, and Quality (ESH&Q) requirements of all applicable laws, regulations and directives. 6.1. Safety Requirements 6.1.1. For off site work, the contractor and its subcontractors shall ensure their personnel work safely and shall be responsible to comply with State and Federal requirements or regulations. 6.1.2. For work performed at the Hanford Site, the contractor and its subcontractors shall be responsible to comply with State, Federal, and DOE requirements or regulations. Where there is a difference in regulations or requirements, the most stringent shall apply. 6.1.3. For visits to the Hanford Site, the contractor shall perform work in compliance with facility-specific procedures and requirements documents applicable to the work area. Contractor will be briefed on any 7 of 22
REQUEST FOR PROPOSAL NO: RMM 180668 TITLE: Demolition Processors and Shear requirements as well as site hazards and the Automated Job Safety Analysis (AJHA) as provided by the buyer. For field visits Contractor shall provide and wear hard hats, safety glasses, high visibility safety vests, and substantial footwear (leather uppers, rubber or similar sole, prefer ankle coverage. No athletic type shoes are allowed). 6.1.4. The contractor shall take appropriate action, up to and including stopping work, and immediately notify the BTR if an unplanned risk or hazard is discovered that is not covered by directions provided by Buyer. The Buyer will then determine whether to modify the safety requirements of this Part 1. 6.2. Quality Assurance and Control 6.2.1 Contractor shall be responsible for performing quality workmanship and shall conduct the quality control measures necessary to ensure work conforms to the specifications. Buyer reserves the right to make inspections at any time at the source of supply of materials. All items and processes are subject to review, inspection or surveillance by Buyer at the contractor’s facility, or any lower-tier subcontractor’s facility.
6.2.2 6.2.3
6.3. Quality Assurance/Inspection Requirements 6.3.1. The following quality assurance requirements are imposed. 6.3.1.1. Procurement of Potentially Suspect or Counterfeit Items 6.3.1.1.1. Contractor shall warrant that “all items furnished under this Contract are genuine (i.e., not counterfeit) and match the quality, test reports, markings and/or fitness for use required by the Contract.” 6.3.1.1.2. The statement shall be on Contractor letterhead and signed by an authorized agent of the Contractor. 6.3.1.1.3. Any materials furnished as part of this Contract which have been previously found to be suspect/counterfeit by the Department of Energy shall not be accepted. 6.3.1.1.4. Suspect/Counterfeit Fastener Headmark lists: 6.3.1.1.5. Fasteners shall exhibit grade marks and manufacturer's identification symbols (headmarks) as required in the specifications referenced in the Purchase Order/Contract Order. 6.3.1.1.6. Any fasteners supplied with headmarks matching those displayed on the following Suspect/Counterfeit Fastener Headmark list, or facsimiles thereof, shall be deemed to be 8 of 22

Fasteners shall be inspected to verify compliance with the Purchase Order/Contract Order requirements. Additionally, fasteners may also be subjected to destructive testing. 6.3.1.1.8. Meetings After contract award, the contractor shall participate in a Project Kickoff Meeting and bi-weekly progress meetings, which may be a conference call, an internet meeting, or a meeting to be held at the Buyer’s Site. The time, date, and agenda for the meeting will be provided to the Contractor by the Buyer. 8. DELIVERABLES, PROJECT CONTROLS, MILESTONES, AND PERFORMANCE SCHEDULE REQUIREMENTS 8.1. Deliverables 8.1.1. Two copies in print and two copies on electronic media of the manufacturer’s spare parts list, maintenance and operations manuals for the processors. 8.1.2. Two copies in print and two copies on electronic media of the manufacturer’s spare parts list, maintenance and operations manuals for the shear. 8.1.3. Two copies of the Material Safety Data Sheets (MSDS) for all grease or oil used in the processor. 8.1.4. Two copies of the Material Safety Data Sheets (MSDS) for all grease or oil used in the shear. 8.1.5. Genuine items warranty statement shall be on Contractor letterhead and signed by an authorized agent of the Contractor (Section 6.3.1). 8.1.6. Two processors in accordance with Section 3.0.

REQUEST FOR PROPOSAL NO: RMM 180668 TITLE: Demolition Processors and Shear PART IV – SPECIAL TERMS 1.0 1.1 ADMINISTRATION Abnormal or Unusual Situations In the event that there is an abnormal or unusual situation associated with this contract work scope, the Contractor is to immediately contact the designated Contract Specialist (CS) or designated Buyer’s Technical Representative (BTR). If the Contractor is unable to contact either the CS or the BTR, the Contractor is to contact the FH Occurrence Notification Center at (509) 376-2900, which is available 24 hours a day, seven days a week, and provide them with: Contract Number, Contract Specialist’s name, BTR’s name and a short summary of the abnormal or unusual situation. If after making contact with FH, the Contractor is advised to suspend activities, the Contractor is not to proceed until such direction to proceed has been expressly issued by the Contract Specialist. If there is an emergency situation, the Contactor is to make the appropriate immediate emergency call to 911 or 373-3800 for cell phones and then make the notifications to FH as set forth herein. 1.2 Designation of Technical Representative The Buyer hereby designates the following as the Buyer’s Technical Representative, (BTR) for this Contract: <>. The BTR is responsible for monitoring and providing technical guidance for this Contract and should be contacted regarding questions or problems of a technical nature. The BTR is also responsible for appropriate surveillance of the Contractor’s representative while on site. In no event, however, will an understanding or agreement, modification, change order, or any deviation from the terms of this Contract be effective or binding upon The Buyer unless formalized by proper Contract documents executed by the Contract Specialist prior to completion of this Contract. On all matters that pertain to Contract terms, the Contractor shall contact the Contract Specialist specified within this Contract. When in the opinion of the Contractor, the BTR requests or directs efforts outside the existing scope of the Contract, the Contractor shall promptly notify the Contract Specialist in writing. The BTR does not possess any explicit, apparent or implied authority to modify the contract. The Buyer reserves the right to inspect electrical equipment and installations. Contractor is responsible for notifying Buyer when installations are available for inspection. Electric motors shall be labeled to be in accordance with NEMA MG-1 or listed by an organization currently recognized by OSHA as an NRTL. Electrical equipment and devices for which there is an NRTL listing category must be Listed or Labeled and/or recognized by UL or another organization currently recognized by OSHA as an NRTL. a. The Canadian Standard Association (CSA) marking is currently recognized by OSHA as an NRTL when the label includes “US” or “NRTL” subscript. b. The European Union “CE” marking, Directive 93/68EEC, is not currently recognized by OSHA as an NRTL marking. c. The International Electrotechnical Commission (IEC) Standard 60529 for enclosures, (IPxx), is not currently recognized by OSHA as an NRTL label. Contractor is responsible for downloading and complying with the applicable revision as identified below. Contractor agrees that the certifications and conditions provided in Representations and Certifications (SP-16) are a material and binding part of, and are hereby incorporated by reference into, this Contract unless specifically excluded and agreed by Buyer in the Contract. Contractor agrees to update and resubmit to the Buyer if any change occurs that would nullify or otherwise affect said representations and certifications.
