PROPOSED BOARD ACTION It is recommended that the State Board grant the Commissioner authority to extend a contract for Cheryl Walker to provide clerical services to the Disability Determinations Section.

BACKGROUND INFORMATION Since February 2, 2004, DDS has contracted with Cheryl Walker for the performance of para-professional services in support of Disability Adjudicators. The contract was originally expected to end on May 28, 2004. The DDS now desires to extend the contract until July 31, 2004. This extension causes the total amount of the contract to exceed $10,000, and State Board approval is therefore necessary.
Estimated Cost
