                          HOMELESS STUDENT COUNT INSTRUCTIONS

EVERY school district is required by federal law to provide count data on homeless students enrolled
                                       during the school year.

                                      Web Survey May 15- June 15, 2009

                                       If you have zero please enter “0”!

DEFINITIONS:

Homeless children and youth: minors who lack a fixed, regular, and adequate nighttime residence. It includes
children and youth who:

   are sharing the housing of other persons (“doubled-up”) due to loss of housing, economic hardship, or a
    similar reason;
   are living in motels, hotels, trailer parks, or camping grounds due to the lack of alternative adequate
    accommodations;
   are living in emergency or transitional shelters;
   are abandoned in hospitals; or
   are awaiting foster care placement;
   have a primary nighttime residence that is a public or private place not designed for or ordinarily used as a
    regular sleeping accommodation for human beings;
   are living in vehicles, parks, public spaces, abandoned buildings, substandard housing, bus or train stations,
    or similar settings; and
   migrant children and youth (as defined under NCLB Title IC – Migrant Education) who qualify as homeless
    for the purposes of this subtitle because the children are living in circumstances described above.

Unaccompanied Homeless Youth: Youths on their own, without supervision by a parent or legal guardian;
runaway and abandoned youth, including youths who are sharing housing due to loss of housing, economic
hardship or a similar reason.

DATA TO ENTER ON THE PRE-COLLECTION FORM:

    1. Count of all homeless children and youth, in families and unaccompanied, that were enrolled in
       K-12 public schools in the district at some point during the 2008-09 school year. These are
       broken down by grade level.

    2. Total number students (included in #1) in any grade that were identified as “unaccompanied
       youth" (see definition above).

    3. Provide a breakdown of the students reported in #1 by the student’s primary residence at the
       time they were identified as “homeless” by the district. The total for #1 will have to match the
       total for #3 on the web survey. Report only one primary residence for each student counted; do
       not record multiple residences for one student.

                              2008-09 DISTRICT PRE-COLLECTION FORM

Collection of the following data is required by the U.S. Department of Education under the McKinney-
Vento Homeless Assistance Act. EVERY school district is required by federal law to provide count
data on homeless students enrolled during the school year, even if enrollment is short term. Use this
form to record the data to be entered later onto the ODE web survey. Complete all blanks, using “0”
when the count is “zero.” The instructions that follow provide definitions and further details.

3. Of the total number of homeless children and youth reported in #1 above, indicate their primary
   night residence AT THE TIME OF THEIR IDENTIFICATION AS “HOMELESS.” The total from this
   section must equal the total from #1 for the web survey to accept the data.
