Question 1: Your A number is assigned to you once USCIS TSC receives you
application for PR. It will be noted on your receipt notice near your name.
Question 3: Class of admission will be your current status. If you have left the US
and entered with advanced parole check your I-94. Your status should be noted as
paroled into the US to pursue permanent residency. If so, your status when you
renew this form will be ADJ.APP – Adjustment applicant.
Question 6: Use your home address – NO PO BOXES
Question 9: If you do not have a social security number, leave blank.
Part 2: Check item d – I am applying for advanced parole document to allow me
to return to the US after temporary foreign travel.

What else do I need to send other than the application form?
You will need to organize you application packet as follows:
1. Attach a check payable to USCIS-Texas Service Center or USCIS Nebraska
Service Center (see below for more on which center to send it to) for $170.00 to
the middle left side of page one of your application.
2. Attach two photos (click here to see sample of required photos) in a small
envelope to the lower left corner of page 1 of the form. Make sure to write your
name in pencil on the back and include your A# if you have one.
3. Behind the I-131 attach the following:
o copy of your I-485 receipt notice if you have one
o copy of the ID page from your passport that shows your photo
o copy of your I-94 card
o copy of any previous advanced parole forms issued to you by USCIS
How and where do I mail my application?
USCIS recently changed procedures on where applications should be submitted. As
of April 1, 2006 all permanent residency applications including I-140 and I-485 are to
be submitted to the Nebraska Service Center (NSC). If you filed your application
before April 1, 2006 with the Texas Service Center (TSC) you will continue to apply
to the TSC for your Advanced Parole. If you filed your applications with Nebraska
after April 1, 2006 you should send your requests for advanced parole to the
Nebraska Service Center (NSC).

If you travel outside the US with advanced parole you MUST re-enter the United
States PRIOR to the expiration date of the advanced parole.
