									            Website Procedures to get an Employer Identification Number Federal Tax ID Number

An Employer Identification Number (EIN), also known as a Federal Tax Identification Number, is a nine-digit
number that the IRS assigns to business entities. The IRS uses this number to identify taxpayers that are required to
file various business tax returns. EINs are used by employers, sole proprietors, corporations, partnerships, non-
profit organizations, trusts and estates, government agencies, certain individuals and other business entities. The
Internal Revenue Service states that all non-profit organizations must have a nine-digit identification number (EIN).
A copy of the annual 990 EZ end of year report must be submitted to headquarters by July 15, each year in order to
avoid chapter fines.

If you have applied for a number and have received one, but have not supplied headquarters with your number then,
you are not listed under the umbrella of Sigma Gamma Rho Sorority, Inc. Please submit your EIN immediately.

If you do not have a nine-digit identification number, please go online to the IRS website to apply for one, complete the items on the SS-4 application. Please follow
the directions below and do not fill in any additional lines.


LINE 1:         “Legal Name of Entity" - Chapter Name then Sigma Gamma Rho Sorority, Inc.

LINE 3:         "Executor...." - Name of Chapter Basileus

LINE 4a:        Mailing Address of Chapter or Current Basileus

LINE 4b:        City, State and Zip of Chapter or Current Basileus

LINE 6:         County and State Where Chapter is Chartered

LINE 7:         Name of Person Completing Report (Must be an Officer)

LINE 8a:        Check the box for "Other Nonprofit Organization."
                Write Sorority on First Line.
                For GEN, WRITE “1540”.

LINE 9:         Banking Purpose (specify purpose) – Open Bank Account

LINE 10:        Give Charter Date

LINE 11:        June

LINE 14:        Community Service Programs

LINE 15:        Education and Health

LINE 16a:       If the answer is yes, please complete lines 16b – 16c

                    Please type the signature, complete phone and fax number on the form.

Within 30 days of receiving IRS response a copy of Employer Identification Number (EIN) letter should be
faxed to International Corporate Headquarters, 1000 Southhill Drive Suite 200, Cary, North Carolina 27513,
