 Save the form in your Word document folder. You do not have to complete it all at once. Simply save your changes each time to exit. You can

 Use the TAB key to move forward between input boxes.



 In the “Financial Information” section, when inputting the dates you will need to input the full year. This will enable you to input information for

more than one year. Example: Input July 2002 as 07/2002 or Jul 2002.



 If you have a problem filling in information in fields containing an underscore, try highlighting that field with your mouse and then typing the

State law requires that all assembly buildings comply with current Life Safety Codes and Handicap Access Codes. State law also

requires that assembly buildings with a span greater than 24 feet or more than 5,000 square feet of enclosed space have a str ucture

designed and sealed by a registered structural engineer and that all assembly buildings costing in excess of $100,000 or seating 50 or

more persons be designed and sealed by a registered architect.

educational building, or mobile chapel of said Church, or for refinancing existing debt related to the church land and facilities , and

for said loan, to execute the Note evidencing same, and a Deed of Trust or its equivalent covering the properties of the Church as

may be by them deemed necessary or desirable in the consummation of said transaction and the completion of the negotiations for said
