							                              Agreement and Receipt for Computer Equipment


       _____________________ [“employee”]             __________________ [“school site”], an employee of the
Baldwin Park Unified School District [“District”], acknowledges receipt of the "laptop" computer, accessories,
and software [“computer equipment”] identified below. Employee agrees to use and maintain the computer
equipment on the following terms and conditions:

1. Use of the computer equipment shall be in strict conformance with Board Policy [“BP”] 4040 and
Administrative Regulations [“AR”] 4040 and 4040.1. Consistent with AR 4040.1 the computer equipment shall
be used for work-related purposes and for limited personal use. Limited personal use is allowed in order to
encourage employees to explore the potential and the drawbacks of the technology, including email, the Internet,
multi-media and other applications. Accelerated Credentialed Educator (ACE) participants use extends to work
required for their Institute of Higher Learning while obtaining their degree or credential.

2. Employee shall at all times operate the computer equipment in a reasonable and responsible manner in
accordance with the manufacturer's instructions. Employee further agrees to safeguard the computer equipment
at all times against loss or damage caused by any reason. Employee agrees to keep harmful products away from
the computer equipment.

3. Employee agrees to be the sole operator of the computer equipment and agrees not to allow any other person
or entity to operate the computer equipment.

4. Employee agrees that he/she has no expectation of privacy concerning the computer and its contents.
Employee agrees to allow the computer to be inspected by a representative of the District upon request.

5. Employee agrees to reimburse District for the fair market value of the computer equipment if it is stolen, lost,
damaged or destroyed, except for theft occurring on District property.

6. At the request of the District and upon separation of employment, employee agrees to immediately return the
computer equipment to his/her supervisor.

7. Employee agrees that any violation of this agreement, BP 4040, AR 4040 or AR 4040.1 shall be cause for
termination.
