Mission Honduras International Team Agreement
1. My team will work only on authorized APUFRAM projects while staying at the Mission. We will at all times follow the direction of the Mission and/or APUFRAM representatives who are responsible for Mission projects. Only disagreements related to safety may be appealed to Father Emil Cook or his representative. 2. I have read and agree with the provisions of guidelines for group leaders listed on the mission’s website. 3. Before departure, each member of my team will have read and acknowledged understanding the volunteer information section provided on the website at www.missionhonduras.com or in the latest issue of the Volunteer Guide. Each group member will agree to abide by the policies listed and implied in this information source. I will ensure that each group member agrees in writing to the conditions outlined above. 4. Our team will meet at least twice to prepare for this trip and will be organized to be reasonably self-sufficient except for room and board while staying at the mission. 5. For teams with healthcare professionals intending to practice at the Mission. All healthcare professionals in my team have read and understand the Health Care Professional Guidelines section provided on the website at www.missionhonduras.com or in the latest issue of the volunteer guide. They agree to abide by the policies listed and implied in this information source. They understand they must provide Mission Honduras International with copies of their medical credentials at least 30 days before their arrival at the Mission. (Credentials must be faxed to the State-side Coordinator at (773) 751-2201.) By my signature on the team application form, or by applying electronically, I consent to the provisions of Paragraphs 1 through 5 of the Team Agreement above. MISSION HONDURAS INTERNATIONAL AND ITS ASSOCIATED PERSONNEL ARE NOT RESPONSIBLE FOR TRAVEL ARRANGEMENTS, LOSS OF VALUABLES, MEDICAL NEEDS OR ACCIDENTS.
