We, _________ and _________, husband and wife, having our residence and permanent domicile in the city of _________, _________ county, _________, each of us being more than twenty-one years of age, of sound mind and disposing memory, do make, publish and declare this to be our last joint and mutual will, hereby revoking all wills and codicils heretofore made by us. 1.
