Instructions for Completing and Processing TAMUK Trademark License Agreement 1. Go to http://www.tamuk.edu/businessoffice/accounting/forms/Instructions_for_Completing_and_Proces sing.doc and download a Word version of the License Agreement.
2. Page 1 1st Unnumbered Paragraph: a. Select Agreement or Renewal (delete the inappropriate term) b. Change date to current date 3. Page 1 Paragraph 1.2: a. Change Company Name b. Change Company Address and Phone Number 4. Page 8 Signature Block a. Complete all Licensee information b. Sign on line titled: By 5. Mail completed form with check to: Texas A&M University-Kingsville MSC 104 700 University Blvd Kingsville, TX 78363 6. Check amount should be for $200.00 US. This includes $100.00 for License Issue Fee and $100.00 for the Minimum Royalty fee. 7. Quarterly Reports a. Quarterly Report are due as per the License Agreement. b. Submit Quarterly Reports and payments as required. 8. Once License Agreement and Initial Payments are received, the University will counter sign agreement and mail copy to vendor. On request, the University will furnish artwork. Vendor shall submit samples or artwork for all products for approval prior to distribution.
