What is CHAP?
CHAP is a King County direct service program assisting families and
individuals to access appropriate health care and social services,
regardless of their ability to pay.

CHAP Demographics
In 2008, CHAP assisted over 6700 families and individuals working
with public and private health providers, giving each client an
average of 2 health and/or social service referrals per call. Call
volume is up close to 50% for the first two quarters of 2009.

Family Resource Coordination
CHAP’S two Family Resource Coordinators (one being the
Lead for King County) connect children (birth-3 years old) to
screenings for developmental delays and provides early
intervention services and support for King County residents.

        CHAP is the point of entry in King County to the WA Infant Toddler Early
         Intervention Program (“ITEIP”), which provides services such as:
            •       Cognitive: ability to learn and how a child learns
            •       Physical: ability to move, see and hear
            •       Communication: ability to understand language and express needs
            •       Social or emotional: ability to relate with others
            •       Adaptive skills: ability to dress, eat and take care of themselves

 Ventanilla de Salud
The Ventanilla de Salud (Health Window) provides linkage to community
resources, screenings and health education for Washington State’s Latino and
Spanish speaking population. Since 2003, it has been operating at the
Consulate of Mexico with the support and participation of the Washington
Health Foundation’s CHAP, Public Health – Seattle & King County, SeaMar and
other community organizations.
                                             The Ventanilla de Salud has helped individuals access
                                              health screenings, health education and community
                                              resources through 1:1 and/or group discussions.
                                             Weekly Topics at the Ventanilla de Salud include:
                                              Screenings and information for medical coverage for
                                              children and adults, women's health care (Pregnancy
                                              Medical, WBCHP and WCHP),vaccinations and connections
                                              to community clinics.
                                             Twice per month the Ventanilla de Salud offers free
                                              preventive health screenings for cholesterol, diabetes, high
                                              blood pressure & HIV testing.
                                             Other topics discussed at the Ventanilla include: Domestic
                                              violence, security in the work place, Alzheimer's in the
                                              Latino community, HIV and much more.
                                             Since December 2008, the Ventanilla de Salud has helped
                                              over 4,000 Latinos in the State of Washington gain access
                                              to health information or services.

Latina Health Fair
For 7 years, CHAP and other community partners have provided low
income or uninsured Washingtonians access to community resources,
health screenings, social and/or medical referrals, outreach, education
and follow-up services through its Annual Latina Health Fair (LHF).
                  The Latina Health Fair has seen a significant increase in the demand for Oral, Blood Pressure,
                   Cholesterol and Diabetes screenings.
                  The 2009 Latina Health Fair served over 800 individuals and families and provided additional
                   services not listed below including free pregnancy testing, developmental screening, vision and
                   body mass index testing.

APPENDIX
Health Coordination Specialist
The CHAP Health Coordination Specialists have been providing
services as ‘Patient navigators’ for many years—part case manager,
part patient advocate and part guardian angel.
        CHAP Health Coordination Specialists are:
            •       A team primarily composed of bilingual Spanish speaking staff with access to Language Line
                    for all other languages
            •       Experts in the areas of Medicaid and other health coverage programs such as Basic Health
            •       Knowledgeable in basic preventive dental care, women’s health, prenatal, child
                    development, social/mental health resources and much more
        Health Coordination Specialists assist individuals with enrollment and/or
         appointments with:
            •       Health care or insurance
            •       Health-screening services
            •       Doctors, dentists, midwives or nurses
            •       Connect clients to Medical or dental clinics with sliding fees
            •       Assist clients in navigating the complex health care system
            •       Other health resources

    A brief look at CHAP’S work & focus:

    Access to Health Care for all Washingtonians
     Since 1986 The Community Health Access Program (CHAP) has been providing Washington State residents
      access to health care.

     CHAP has assisted over 124,000 families and individuals with medical, dental, and specialty care services regardless of their
      ability to pay through a toll free phone line.

     Our focus in the community has been to create awareness and education about preventable diseases such as, but not limited
      to: Breast, Cervical, and Colon Cancer.

     CHAP along with its Family Resource Coordinators has connected many children birth-3 years of age to developmental
      screening through the WA Infant Toddler Early Intervention Program.

     Aside from providing social and medical assistance over the phone, CHAP has left a physical imprint in the community
      through programs such as the weekly Ventanilla de Salud and its annual Latina Health Fair, which provide individuals with
      outreach, referrals and free health screenings in a trusted and compassionate environment.

     CHAP partners with Public Health - Seattle & King County to provide free medical assistance trainings throughout the year to
      other community partners.

     CHAP participates in the planning and running of the First Friday Forum group that provides a forum for outreach workers and
      other agency staff working in Medicaid and other state health coverage programs to problem solve and be kept up to date on
      changes in the different systems

     CHAP serves on various committee’s and workgroups related to health access to provide a voice for our clients.

     At CHAP, our mission over the years and today has been to sustain healthy people and healthy communities by ensuring that
      all Washingtonians have access to timely and appropriate health care and resources.
