                   Draft Notification Process in Response to a Child Death


                                                          If death occurs at a Hospital:
         If death occurs outside of a                         Appropriate professional to contact

 * NB: Deaths that require Rapid Response
 Deaths triggering a Rapid Response will be unexpected deaths that require immediate
 information sharing e.g. Suicides; any suspicion of abuse/neglect.

 Medically expected deaths or deaths where a full and sufficient medical cause is immediately
 available do not require Rapid Response, but will need to be ‘notified’ to Child Death
 Administrator.
