included as a guide. Parts (a) and (b) are not shown as they relate to data collected at Local Authority
level. If possible, please provide data for (c) to (s). You may however, for reasons of confidentiality,
prefer to provide only (c), (d), (m), (n), and (o). This is the minimum data required to enable the
calculation of an accurate figure for take up at Local Authority level.
