									     Assessment and Treatment of ‘Fragile’ and Personality Disordered
        Patients with Intensive Short-Term Dynamic Psychotherapy

Fragile patients experience chronic somatic (e.g., nausea, headaches, fatigue, IBS),
cognitive (e.g., dissociation, “blanking out”, mental confusion, concentration problems)
and behavioural (e.g., weepiness, tantrums, collapsing, and other “acting-out” behaviours)
symptoms and frequently present with diagnoses of borderline and other personality
disorders, panic disorder, and chronic depression.                                           They tend to respond poorly to
therapy, but comprise up to 20% of referrals to a private practice (Abbass, 2002).
Fragile patients often have frequent cycles of hospital admissions and changes in
medications without lasting effect on their symptoms.

Intensive Short-Term Dynamic Psychotherapy (ISTDP) is an attachment theory-based, brief,
focused, empirically validated, interpersonal-style therapy that works with a patient’s
anxiety, defenses and underlying emotions via situations in the patient’s current life,
including their moment-to-moment experience with the therapist.                                                      The specific and focused
framework within which the therapist works enables a rapid restructuring of fragility and
the building of resilience in a short period of time.
Dr Stephen Arthey is the former senior clinical psychologist at a large private
psychiatric hospital in Melbourne where he developed and managed treatment programs for
personality disorders, chronic depression, PTSD, eating disorders and anxiety disorders.
He has trained in ISTDP with its founder, Dr Habib Davanloo, and other leading ISTDP
clinicians.            Dr Arthey has been practicing ISTDP for over 12 years and is now in full-
time private practice treating adults and providing supervision to psychologists.

IN THIS ONE-DAY WORKSHOP, YOU WILL LEARN:

    How to precisely diagnose the level of a patient’s fragility;
    To accurately determine into which channels (Somatic, Cognitive, Behavioural) they
     discharge their anxiety;
    To understand the underlying psychopathology of fragile and personality disordered
     patients;
    How to effectively and rapidly restructure fragility, eliminate regressive behaviours
     and build resilience.
                 Each aspect of the restructuring treatment will be demonstrated with
             Video-recorded extracts from therapy sessions with highly fragile patients.
                                        Prior knowledge of ISTDP is not required.
