MS 2600                         BASIS FOR
                      TREATMENT CENTER PARTICIPATION

  [In order for the residents of Drug Addiction and Alcohol (DAA) treatment
  centers to be eligible to receive food stamp benefits, the DAA center must:

  A.   Be certified by the Food and Nutrition Service (FNS) as an authorized
       retailer to accept the EBT card. To determine if the DAA center is an
       authorized retailer, contact FNS at (859) 233-2411; OR

  B.   Provide evidence that it meets one of the following qualifications:

       1.   Is tax exempt; and
       2.   Certified by the State agency responsible for the treatment and
            rehabilitation of drug addicts or alcoholics (the State Title XIX Agency)
            as:

               Receiving funding under part B of Title XIX; or
               Eligible to receive funding under part B of Title XIX even if no
                funds are being received; or
               Operating to further the purpose of part B of Title XIX, to provide
                treatment and rehabilitation of drug addicts and/or alcoholics.

  Facilities that are certified by the state as an alcohol or drug treatment center
  can be found at http://chfs.ky.gov/os/oig/directories.htm. After opening the
  page, the list of certified DAA centers can be found under “Alcohol Drug Entity
  Directory”.

  Some DAA centers, such as recovery programs, are not monitored by the
  Cabinet. These facilities are not certified and are not included on the above list.
  However, the facility may still be operating a program “to further the purpose of
  part B of Title XIX”. Residents of these facilities may be eligible to receive
  benefits if the DAA Centers:

       Provides a letter from the Cabinet for Health and Family Services, Dept. for
       Mental Health, Developmental Disabilities and Addiction Services
       (DMH/DDAS), which states the facility is operating to further the purpose of
       part B of Title XIX, to provide treatment and rehabilitation of drug addicts
       and/or alcoholics. To obtain this letter, DAA centers must call DMH/DDAS
       at (502) 564-4456. The facility will have to supply DMH/DDAS with a list of
       services that they provide to their residents in order to determine if the
       facility qualifies for the above-mentioned letter.

  Applicants/recipients who are residents of facilities that are unable to provide
  proper documentation are not eligible to receive food stamp benefits until
  required documentation has been provided.
