Massanutten Learn-to-Ski Package Sales
Get a jump start on the purchase process, print pages 2 & 3 of this document, fill it out, and bring it with you to our office at 155 W. Davis Street (right next to the Fire Hall in downtown).
Remember, each person has to have a separate form, and only one package can be purchased on a single form. We apologize for this inconvenience, but it is how the paperwork must be submitted. Don’t forget to sign page 3. We cannot accept the form without the signature.
Payment methods: Cash or check only. We cannot accept credit or debit cards. Checks are preferred, and must be made payable to “Culpeper Co. Parks & Rec.” Packages go on-sale Monday, December 1st at 8:00am. The office will stay open until 8:00pm on this day only for the convenience of people who work out of town. Packages are only sold in person. We will not process any forms that come by mail, nor can we process sales over the phone.

The approved teaching progressions utilized in this program are designed to enhance effective learning during the class group lessons and improvement during practice exercises. I will follow Your Responsibility Code (as shown in the Massanutten License-To-Slide Program brochure, ski rates brochure, and as posted at various locations around the ski area) at all times, including “out of class” practice periods without direct supervision of instructors.

THE FOLLOWING APPLIES TO ALL PARTICIPANTS:
Massanutten Resort will not refund any monies or make any type of restitution unless the Resort does not offer all six sessions. There is no guarantee that the resort will be open after, or up to, any specific date. I have read and understand all the information in this License-to-Slide Program brochure. I understand and accept the fact that alpine skiing in its various forms is a hazardous sport that has many dangers and risks. I realize that injuries are a common and ordinary occurrence of this sport. I agree, as a condition of being allowed to use the ski area facility and premises, that I freely accept and voluntarily assume all risks of personal injury, or death, or property damage, and release Massanutten Ski Area and its agents, employees, directors, officers, and shareholders and facilities, the operations of the ski area including, but not limited to, grooming, snowmaking, ski lift operations, actions or omissions of employees or agents of the area, or my participation in skiing and snowboarding or other activities at the area, accepting myself the full responsibility for any and all such damage or injury of any kind which may result.
CONDITIONS OF USE. I, the undersigned, have read and understand the terms of the above License-to-Slide Liability Release
Agreement, which is an essential part of it. I am signing it freely and of my own accord, realizing it is binding upon myself, my heirs and assigns, and in the event that I am signing it on behalf of any minors, that I have full authority to do so, realizing its binding effect on them as well as myself. As a condition of holding a License-to-Slide card and being permitted to use any and all of the facilities of the ski area, I further agree to assume all risk of personal injury or loss or damage to any of my property. I understand that attendance at the first lesson is mandatory to be in any lesson.
County of Culpeper Assumption of Risk and Release: In agreeing to participate in the program, as an adult participant or as a parent or a guardian of a participant, I and/or the participant do hereby affirm that the general health of the participant is good, and that the participant is not adversely affected by exercise, and that the participant is capable of performing an activity of this nature.
In consideration of participating in this activity, I and/or the participant do hereby assume all risk of any injury to the participant and will indemnify and hold harmless, from any and all liability, action, cause of action, claims, and demands of every kind or nature whatsoever that I and/or the participant have or which arise from or in connection with my participation in this activity, the County of Culpeper, Virginia, the County Board of Supervisors, the Culpeper County Department of Parks and Recreation, the Culpeper County School Board, the Town of Culpeper, and all their officers, agents, employees, staff, volunteers and successors. It is likewise assumed and agreed that the participant will, at his own expense, wear the proper clothing and protective equipment during the activity, and that it is the responsibility of the participant or parent or guardian to make sure the criteria are met. I grant my permission to the county or its agents to transport the participant to and from an event or activity when required and hold harmless those assigned to transport. I also agree to allow transportation of the participant to the nearest physician or hospital for medical treatment and agree to allow for immediate first aid to the injured participant, if and when deemed necessary.

If you miss two consecutive lessons, you will not be eligible for any License-to-Slide classes for the remainder of the season. Your slope use ticket and rental punches will remain valid.
