Notes on completion of Welsh Masters Stillwater Championships Entry Forms 2010
These forms will only be accepted if completed electronically and E-Mailed as described.
Hard copies will not be accepted
NB Cells shaded grey are protected cells and data cannot be entered into them
Follow the instructions given below and format the data you input as stated to ensure that the required information records
accurately. If you do this the number of competitors entered for each event and the total fees to be paid will be displayed at the top
of the page
1 Save this file in your My Documents folder as: Welsh Masters Stillwater Entry 2010 xxx.xls where xxx = the name of your club
Locate the file in your My Documents folder and begin entering data. Switch on your CAPS LOCK to ensure that data is entered in capitals.
Check that the spelling of names entered corresponds to that sent with the membership details that are registered on the SLSA Membership
2 database as all entries will be checked for validity against this information. Please do not use nicknames as these cause confusion! Enter
details of all competitors taking part in the Competitor Details sheet as follows (an example is provided in the shaded cells):
a) Column A, if you are a NON SLSA WALES club enter your two-letter club code, SLSA WALES clubs please leave this column blank
b) Do not enter anything in column B - (this cell is protected and you will get an error message if you try to)
Column C, if you are a SLSA WALES club, enter the competitors MIS number, if you are a non Welsh club do not enter anything in

column C as this will be filled in automatically
d) In column D enter the forename of the competitor
e) In column E enter the surname of the competitor
In column F enter the sex of the competitor - either M for Male or F for Female (ensure that you use only these two letters so that the

automated total at the top of the page is calculated)
Do not enter anything in column L - this will complete automatically with the correct age group once column N has been completed. If

column L remains blank this means the competitor is too young ie born on or after 01/01/1980 - this cell is protected also
In column N enter the competitor's Date of Birth in the --/--/-- format. Note that the automation of columns L and M will not work if this

format is not used
In columns O to S, having checked that the competitor is not too young, insert an entry time for each of the individual events into the relevant
column for that competitor. You must enter a correct time for each event being entered using the format --:--.-- If this format is not used the

entry will not be totalled at the top of the page, nor the entry fee included in the total. If the competitor does not have a valid entry time but is
being entered into the event, insert 10:00.00 *THIS FORMAT MUST BE USED*

3 Enter basic details of teams taking part in the Team Entries Sheet (an example is provided in the shaded cells):
a) In column A enter your two letter club code followed by TA, TB, TC, TD etc for each team participating in different events
b) In column B enter your club name followed by Team A, Team B, Team C etc
In column C enter the sex of the team - either Mixed, M for Male or F for Female (ensure that you use only these letters and words so that

In columns E to F insert an entry time for each of the Team events into the relevant column for that team. You must enter a correct time for

4 When you have finished completing all of the entry information, enter the relevant details on the Club Declaration sheet to confirm:
a) Name of the Club that is being entered
A declaration that the Team Manager will bring a cheque to cover the full amount of entry fees shown opposite to the registration session, and
b)
acknowledgement that until the cheque is received the club will not be permitted to participate.
A declaration that all of the competitors entered are paid up members of SLSA Wales or a recognised NGB for the 2010 year and can all

swim 50m without assistance
Acknowledgement that if competitors entered are not paid up members of SLSA Wales or a recognised NGB for the 2010 year, their name will
d)
be deleted from the draw, but entry fees will still be payable
Name of the official Timekeeper(s) that the Club will provide (all timekeepers are to report to the Referee and be on poolside by 8.45am on
e) the day of the competition. If more than one is being supplied, please include all names. All Timekeepers need to bring a stopwatch with
them.
Name of the person who will assist in towing manikins back to the starting position during the heats of events. This person will need to listen

for PA announcements which will inform them when and where to report
e) Name of the Team Manager, his/her contact telephone number, and E-Mail address - the draw sheets will be E-Mailed to that person
f) Confirmation of the number of manikins and rescue tubes that the club will be bringing to the event
g) Your contact details and position within the club as the person responsible for completing these entry forms
Finally, ensure that you save the information in the spreadsheet before transmitting via E-Mail to arrive no later than
18.00 Wednesday 27th January 2010. You must submit your entries to Jon Gwynn, at

Please remember that the date for age qualification is 31st December 2009. Competitors must be at least 30 years of age on that

This is the entry for the Welsh Masters Stillwater Championships 2008 for our Club. I hereby certify that the details
given on the attached Competitor List, and Team Entry forms are correct to the best of my knowledge and belief. I
acknowledge that a cheque, made payable to SLSA Wales in respect of the amount shown above, is due for these
entries, and that this will be brought to the Registration Session in order for our club to be able to participate in
these Championships.
2 All competitors listed on these forms are fully paid up registered individual members of the Surf Life
Saving Association of Wales at the time of lodgement of these forms and all are able to swim at least
50m without assistance.
3 I understand that if it is subsequently discovered that any competitor listed is not a fully paid up
registered individual member of the Surf Life Saving Association of Wales, his/her name will be
deleted from the draw and no refund of entry fees will be given.
4 The following person(s) have been nominated by our club to act as official Timekeepers for the duration
of the event and will report to the Referee on poolside by 8:45am on the day of competition. I
understand that failure to provide an official Timekeeper will mean that our club will not be cleared to
compete in these Championships.
