What is a valid forfeit claim?
If you attend your appointment and only one or no teams turn up to the ground then you are eligible to make a forfeit claim.
If you have been notified by our appointment officers that the game has been cancelled or forfeited then you are not eligible for a
forfeit claim.

Games called off due to wet weather, even if you attend the oval are not regarded as a forfeit and no claim can be made.


TO COMPLETE FORM
1. This form can be computer generated and must emailed to admin@ccfootballref.com.au or hand written and mailed to Administration Officer PO
BOX 5095 ERINA FAIR 2250.
2. Enter your name and date submitted in the relevant area. i.e. 21st of February 2009 would be entered as 21/2/09
3. Ignore the claim number column as this is for office use only.
4. Enter the date the forfeit actually happened.
5. Enter the ground at which the forfeit occurred.
6. Enter the grade that you should have refereed. i.e. 16B, 11C or PL1 RES.
7. Enter the home team and the away team in the next columns.
8. Where you the Referee or Assistant. Please state as REF or ASSIST
9. Enter the fee you expected to receive for doing the stated game.
10. VERY IMPORTANT. Give details of why there was a forfeit. i.e. Away team didn't show up for the kickoff time or only 6 players were present at kickoff time.
The more details you can give the better, but keep them brief as possible.
11. Send this form to the ADMINISTRATION OFFICER at the above mentioned email or postal address.
12. The Administration Officer will colate the forfeits on a monthly basis and forward them to the Financial Director.
13. The Financial Director makes the claim on Central Coast Football and then pays the relevant referees.
14. You must pay the levies on these games as the Financial Director will reimburse you for the full amount that youn should have received.
