to gather with him in Madrid Spain in August of 2011.

What Is World Youth Day?
Conceived by Pope John Paul II in 1984, World Youth Day celebrates the opportunity for
young people from around the world to deepen and renew faith, broaden perspectives and
nurture the human spirit. WYD gatherings make it possible for young people from
different lands and cultures to meet together, share diverse perspectives, and gain an
appreciation for challenges - both similar and unique - to develop a faith that is authentic,
lively and respectful. Above all it is a celebration of joy in the Lord!
WYD MADRID PACKAGE INCLUDES:

ADULT LEADERS: Adults interested in helping lead this Pilgrimage must be
practicing Catholics and part of St. Mary’s Worshiping Community. One adult to
every five teens will be needed. Preference will go to adult leaders already assisting
in St. Mary’s Youth Ministry. To apply, please give your name to Matt Chick. Adult
Leaders will be chosen in early February, 2010.
YOUNG ADULT PARTICIPANTS: Young adults 18 to 25 should turn in the same
form as minors, but no parental signature is needed.

After deposit is made, the following payment
schedule will apply:

[St. Mary’s teens will have many fund-raising opportunities to help defray the WYD Madrid costs. Some

The initial deposit will be non-refundable. From February 1, 2010 – July 31, 2010, a total
cancellation fee of $500 penalty will apply. From August 1, 2010 – January 14, 2011, a penalty of
$1000 will be incurred. From January 15, 2011 – March 30, 2011, a $1500 penalty will be incurred.
After April 1, 2011, all payments are non-refundable. It is for these reasons, travel insurance is
highly recommended. All cancellations must be submitted in writing. Prices are based on euro to
dollar conversion as of July 2009 and are subject to change based on the conversion in 2011.
Dube Travel Agency, Inc., and representatives of Dube Travel act only in the capacity of agents for
the tour participants and for the various vendors including airlines, transportation companies, hotels,
sightseeing, restaurants contractors and all other individuals performing duties as specified in the
proposal and tour flyer. Dube Travel will exercise reasonable care in making arrangements,
however, does not assume any liability whatsoever for any injury, damage, loss, accident or delay to
person or property. The tickets and contracts in use by any carrier, hotel, or other contractor
rendering services shall constitute the sole contract between such contractor and the tour member.
No refund will be made for any unused portion of the tour unless agreed to in writing prior to
departure of the tour. The right is reserved to decline to accept or retain any person as a member of
the tour at any time. The right is reserved to change or cancel itineraries, substitute services with or
without notice where deemed necessary. The issuance and acceptance of tickets and tour documents shall be
deemed to be consent to the above conditions. The air carrier and all other transportation vendors shall not be
held responsible for any act, omission, or event during the time passengers are not on board its own
A valid US Passport is required for travel to Spain. Please note the expiration date must be more
than 6 months from the date of your return. Please inquire with Dube Travel if you have any

I hereby request to participate in the St. Mary’s Pilgrimage to World Youth Day in Madrid,
Spain. I will be 16 or older by August, 2011. I recognize that this is a spiritual journey that
begins as soon as I sign this form. I hereby commit to the following as part of my pilgrimage
1) I will practice my Catholic faith by worshipping every weekend at Mass, by a private
daily prayer life, and by striving to love God and neighbor as Jesus taught.
