Project STAR AmeriCorps Performance Measure Worksheet Issue Area: Human Needs General Info Performance Measurement Title: Imagine Project Mentoring Measure Category: Needs and Service Activities ALIGNED PERFORMANCE MEASURES SAMPLE: MENTORING Service Category addressed by this Performance Measure Worksheet: Intensive Mentoring Needs and Service Activities Briefly describe the need to be addressed (4,000 characters or less): Large numbers of youth in the community, left unsupervised, have become involved in gangs and other destructive activities, according to the Office of Juvenile Justice Report, 2005. Low self-esteem and boredom also contribute to poor decisions. These “at-risk” youth can benefit from the guidance of a mentor and participation in goal-oriented activities that teach them useful skills and hold their interest. Briefly describe how you will achieve this result (4,000 characters or less): Twenty members will mentor 20 “at-risk” youth, ages 14-18, individually. Mentors and youths will meet once a week, on Saturdays from 10 a.m. to 3 p.m., for ten months (November 2006 through August 2007), and work as partners in the Imagine Improvement Project, to build and repair housing for low-income families and seniors, and to maintain the Imagine Improvement Project Community Garden. Mentors will serve as role models to youths, who will gain new skills, responsibility, and self-efficacy. How many AmeriCorps members will be participating in this activity? 20 How many days per week (on average) will this activity occur? 1 How many hours per day (on average) will this activity occur? 5 When does this activity begin? 11/20/06 When does this activity end? 8/14/07 Results The outputs and outcomes you intend to track for a particular activity: Result Type Outputs are counts of the amount of service members or volunteers have completed, but do not provide information on benefits to or other changes in the lives of members and/or beneficiaries. Intermediate-outcomes specify changes that have occurred in the lives of members and/or beneficiaries, but are short of a significant benefit for them.
