TERMINATION NOTICE Date: [DATE] To: [RECIPIENT] From: [SENDER] Please be advised that your employment with the Company will end on [DATE]. On [DATE], we met and reviewed your unsatisfactory work performance rating and discussed the steps you could take to correct the problem. That discussion was summarized in an Unsatisfactory Work Performance Memorandum dated [DATE] a copy of which is attached to this notice. Despite our efforts, there has been no meaningful improvement in your performance and we have concluded that you must be terminated from employment for the following reason(s): Because you are being terminated for cause, you are not entitled to severance pay. At the time you receive your final paycheck, you will also receive a letter advising you whether and to what extent you are entitled to any accrued benefits including, but not limited to, continued medical and life insurance coverage and retirement benefits.
