                   8.   Duties
                        a) Provides the facility with appropriate information regarding applicable pharmacy/medication
                            federal/state statutes, rules, or regulations
                        b) Ensures safe storage of all controlled substances and other medications in the facility
                        c) Acquires and maintains facility’s registration with the State Board of Pharmacy and DEA
                        d) Establishes requirements for purchase and storage of all materials, including drugs,
                            chemicals, and biologicals
                        e) Participates in the development of a formulary, subject to approval of the governing body
                        f) Assists in the implementation of the policies and decisions of the performance-improvement
                            committee relating to pharmaceutical services of the facility
                        g) Continually evaluates the distribution of drugs to be administered to patients according to an
                            original or direct copy of the practitioner's medication order

                          h)Maintains and makes available a sufficient inventory of antidotes and other emergency
                            drugs, as well as current antidote information, telephone numbers of regional poison control
                            center, and other emergency assistance organizations, as deemed necessary by the
                            performance-improvement committee
                        i) Maintains records of all transactions of the facility as required by applicable state and federal
                            law, and as necessary to maintain accurate control over and accountability for all
                            pharmaceutical materials
                        j) Participates in those aspects of the patient-care evaluation program that relate to
                            pharmaceutical-material utilization and effectiveness
                        k) Participates in teaching/research programs in the facility
                        l) Provides and updates information for polices and procedures for obtaining, storing,
                            administering, and disposing of all medications within the facility
                        m) Reviews and updates medication policies and procedures to meet state and federal laws
                            annually
                        n) Continually evaluates the labeling, storage, and distribution of investigational drugs, including
                            maintenance of information in the pharmacy and nursing station where such drugs are
                            administered, regarding dosage form, route of administration, strength, actions, uses, side
                            effects, adverse effects, interactions, and symptoms of toxicity
                        o) Performs admixture of parenteral products; ensures education and training of nursing
                            personnel concerning incompatibility; provides incompatibility information when admixture is
                            not performed in facility
                        p) Conducts the required bi-annual DEA controlled substances inventory on the original date, or
                            within five days following the original date
                        q) Checks the controlled substances ledger monthly for accuracy
                        r) Works with the administrator/clinical director in conducting a check of medication procedures,
                            including refrigerator temperatures, out-of-date drugs, crash cart inventory accuracy, security
                            of the controlled substances storage area, and other items on a monthly basis
                        s) Reports to the administrator/clinical director any problems related to pharmaceutical
                            deficiencies, errors, or inaccuracies. These problems will be reviewed and evaluated by the
                            administrator and consultant pharmacist, and any actions taken will be reported to the
                            governing body and proper authorities
                        t) Participates in meeting all inspection and other requirements of pharmacy laws and rules
                        u) Evaluates and assists with implementation of record maintenance
                        v) Reviews and evaluates maintenance of controlled substances (e.g., ordering, storage, and
                            documentation of use)
                        w) Provides review and inspections at least monthly of medication areas and carts to identify
                            and handle outdated or discontinued medications and handling and documentation of
                            controlled substances
                        x) Advises staff regarding policies and procedures for using hazardous substances
                        y) Recommends methods for upgrading patient care through the drug distribution system
                        z) Provides nursing staff with periodical memoranda concerning drugs and drug-related items
                        aa) Performs semi-annual review of all aspects of the total drug distribution system
                        bb) Is available to the facility on an as-needed basis in addition to regularly scheduled
                            consultations

Requirements
                     Required
                     1.   High school graduate or GED certificate
                     2.   Degree in pharmacy, Rph
                     3.   Pharmacist’s license in appropriate state
                     4.   Consultant pharmacist’s qualifications as set forth by federal and state statutes, rules, or
                          regulations
                     5.   Strong ethical and moral character references
