             Follow these “Getting Started” steps to prepare your scholarship application. Use the worksheet to keep track of
             scholarships and prepare other required materials such as your Activities Chart, essays, and transcripts. Instruc-
             tions for completing the worksheet and eApp begin on page 29.

The Oregon Scholarship eApp Worksheet allows you to read and complete the questions before entering your information online.
The questions are listed in the same order as they appear on the eApp web site.

DO NOT MAIL THIS WORKSHEET! This worksheet is optional and should not be mailed to the Oregon Student Assistance Commission
(OSAC) as a completed scholarship application. Complete and submit your eApp online at www.GetCollegeFunds.org. Applying
online is faster and easier than using a paper application. The eApp also helps you by prompting you to complete required fields so
you are less likely to make mistakes. Last year more than 92% of students submitted an eApp.
Students who cannot access a computer to submit an eApp can contact OSAC at 800-452-8807 to request a paper application or
download a .pdf at www.GetCollegeFunds.org


  1 Identifying Information

 Your legal name (first, middle initial, and last name). Make sure it is
 the same name used when filing your FAFSA.
