					UK Permits and Recruitment
PLEASE READ BEFORE COMPLETING THIS FORM:

Please fill out all sections of the form below ACCURATELY and CAREFULLY as any mistakes will
slow down and may even invalidate your application. A small explanation of what information is
required is given under each question. To answer the questions simply click in the middle of the relevant
box and begin typing. Then save the document, with your answers filled in, and attach it as a file to an e-
mail.

Although this form seems long, it is THE ONLY FORM you will have to fill in for the duration of the
process as the rest of the details will be obtained from your CV. Besides, many of questions require
simple YES/NO answers and the form should take no longer than 30-40 minutes of your time. If there are
any technical problems in filling in, saving or attaching the form, you can print off the form and fill it in
by hand and post it to our offices (address on the website). Be sure to include an e-mail address with the
letter, as this is our primary means of contact before the application is sent.

If you then need to fill in another form for one of your DEPENDANTS, after having saved your
own filled in form as the main applicant form, simply go back to our ‘Contact Us’ page and
download another form. Then fill it in accordingly for your dependant and save it as a separate
document. If you have more than one dependant simply repeat the process, saving each form as a
separate document which then must be separately attached to the e-mail. When you have filled out
all the forms relevant for your application attach them all to an e-mail and send along with your
CV.

After receiving this information we will be in a position to not only assess your eligibility for the scheme,
but begin working on your case. We will then correspond with you to inform you whether or not you
meet the 75 points requirement, and to offer you our services for then completing your entire application,
at which point you will hopefully choose to continue with us. UK Permits and Recruitment offer the

whether you used the visa or not


Have you ever been refused a visa for
any country including the UK?
If YES please provide details. If you were refused a
visa for the UK please enter either your Home
Office reference number which is prefixed with the
first letter of your family name, for example Smith
= S12345 or your UKvisas reference number in the
following format XXX/123456 (the XXX is the
country or post location abbreviation, for example
New York is NYC).

arrival to the UK?
If YES please provide details. You must tell us if
you have ever been refused entry to the UK once
you arrived at Immigration Control at a port or
airport. Or if once in the UK your visa/Leave to
Remain was cancelled and you were required to

If YES please provide details. You must record
ANY convictions you have obtained during your
lifetime in any country including the UK.
Convictions will not necessarily result in automatic
refusal. However failure to disclose any conviction
will be taken into consideration. If you have any
convictions you must submit the official record of
the conviction, detailing the crime you were
convicted of along with any sentence received
when you come to hand in your final application.
