A description of the services provided by each party, Church View Medical Centre is a GP Practice with a list size of
including products and services that both parties approximately 6300 patients based in Silksworth, Sunderland. It offers a
supply and any areas of geographic overlap; traditional range of GP services as specified in a PMS contract as well as
offering additional enhanced services supported by the South of Tyne &
Wear PCT and in line with local patient requirements. Church View
Medical Practice is very similar to many other GP Practices based in the
locality and across the whole of Sunderland.
City Hospitals Sunderland NHS Foundation Trust is a large District
General Hospital based in Sunderland comprising approximately 1000
beds within an annual turnover approaching £250m. As well as providing
all the local acute services that most District General Hospitals would
provide, City Hospitals also provides some services such as
Ophthalmology, Urology, Head and Neck and Renal Dialysis services on
a regional or semi regional basis.
Although the geographic service provision areas overlap with City
Hospitals Sunderland being much larger than, and encompassing, Church
View Medical Practice, there are no significant services that both parties
supply, as in they are in different service/market sectors.
Church View Medical Practice is located in Silksworth and there are five
similar practices within a one mile radius. There are two medical
practices (Dr Joshi and Colliery Medical Group) based at Silksworth
Health Centre which is approximately 50 yards from Church View Medical
Centre. In addition there are four further surgeries within a one mile
radius and these include Dr Weaver, Springwell House, Southlands
Medical Group and Peace Haven.
There are five hospitals ranging within 7 and 17 miles of City Hospitals
Sunderland with the closest being South Tyneside NHS Foundation trust
and the furthest being Newcastle General Hospital which is part of the
Newcastle Hospitals NHS Foundation Trust.
Approximately 95% of all Church View’s referrals to secondary care are
directed through City Hospitals and this situation is not dissimilar across
most of the other practices in Sunderland. The main reason that GP
practices refer out of Sunderland is because the hospital does not provide
the specific service required. This tends to be for specialised or tertiary
specialities such as cardio thoracic surgery, radiotherapy etc.
On this basis any patients affected by this change can easily transfer to
other local GPs who all have open lists.

The value and nature of the consideration provided for There is no consideration involved as this will be based around the
the proposed novation; novation of the existing PMS contract.
The rationale for the novation. The aim of the novation is to remove organisation and contractual barriers
in order to improve patient care. This will facilitate clinical integration and
improve communication between primary and secondary care to help
prevent avoidable admissions, facilitate discharge and help prevent re-
admission in our target population within the ICO Pilot. The proposal will
explore new models of working together to deliver improved outcomes
due to active management of patients with long term conditions leading to
more holistic care and improved quality of life for patients.
We feel that the pilot will help ensure full utilisation of resources available
across primary and secondary care by reduction in the use of unplanned
out of hour’s services A&E, Emergency Admissions and Re-admissions.
Also we hope to extend patient lives and improve “the staying healthy”
agenda to delay patient’s transitions to frequent fliers as well as
supporting dignified end of life care.
This proposal has been selected as one of sixteen Department of Health
sponsored projects to investigate the costs and benefits of integrated
care. This will require continuous analysis, evaluation and reporting both
on an internal and external basis with independent scrutiny from the DH,
Rand and KPMG. Also as a key stakeholder the PCT will be reviewing the
impact on choice and supplier induced demand in line with PRCC rule 10.
The stated aim of the project is to reduce hospital admissions and if
successful in the target cohort is likely to be rolled out for more segments
of the practice patients.
The aim of the pilot is to demonstrate a significant reduction in unplanned
interventions for patients over the longer term.
