									Dear Parents/Guardians,

Flu season is nearly here, and children are twice as likely to get the flu than adults. Children can be sick
with the flu for a week or more, which may cause them to miss school. In fact, in 1996, the flu caused
U.S. children to miss 38 million school days. Additionally, flu in children can be serious and can cause
fever, headache, fatigue, and body aches.

This season, USD 501 school nurses, with help from the Shawnee County Health Agency, would like to
help protect K-12 students in Topeka Public Schools against the flu by offering flu shots at school.
These flu shot clinics are strictly voluntary.

A schedule of the flu clinics can be found on the back of this letter.

There will be no charge for children with Medicaid/Unicare/Family Health Partner cards. The child’s ID
number from the card should be provided on the Flu Vaccine Administration Record and Encounter. For
all others, the cost for the flu shot is $20.00. Payment can be made in one of the following ways:
      Cash
      Check made out to the Shawnee County Health Agency
Those with private insurance will be given a receipt after the shot is given, which you can submit to your
insurance company.

If you would like for your child to participate:
      Read all the materials carefully
      Complete and sign the Flu Vaccine Administration Record and Encounter
      Return completed form for each child, along with payment, to your school nurse

Students with incomplete paperwork or missing payment will be unable to receive a
vaccination.
