Eligibility Requirements
1. Applicants must be members of the NATA and a District 8 member of the NATA prior to February 1st,
2009.
2. Applicants must have an overall GPA of 3.0 (based on a maximum of 4.0) for ALL undergraduate
courses to date, to include the Fall 2008 semester.
3. Applicants must have performed with distinction as a participant in his/her athletic training program,
academic major, institution, intercollegiate athletics, or American higher education. NOTE: The degree
of the student's athletic training achievement shall be weighed at least equally with the degree of their
athletic training performance.
4. Applicants must signify their intention to continue academic work to the baccalaureate degree as a full
time undergraduate student and have been judged capable of this study by their major professor, major
department head, or dean of the college in which they have enrolled.
5. It must be the intent of the recipient of the scholarship that they pursue the profession of athletic
training as their means of livelihood.
6. After satisfying the above requirements, consideration shall be given to their participation in campus
activities other than academic and athletic training in which they have had the opportunity to
demonstrate their qualities of leadership and serve as an example to their fellow students.
Application Instructions
1. All sections of the application packet must be fully completed and signed as directed

Section II Nomination Form (to be completed and signed by a BOC certified athletic trainer, who
can attest to the applicant’s skills, abilities and scholarly activities as they relate to this
application)
Section III Institutional Endorsement (to be completed and signed by the dean of the college or the
department head responsible for the applicant’s academic program)
Section IV Applicant’s Essay (to be written and signed by the applicant)
2. All information in the application packet (other than signatures) must be typed
3. Application packets must include official transcripts of ALL undergraduate course work to date, to
include the Fall of the 2008. Incomplete application packets will not be processed. Deadline for filing
completed applications is (postmarked by) February 10, 2009
