Instructions for Completion and Submittal As a supplemental attachment to the Parish Recovery Proposal template, the LRA/OCD-DRU team has created this Parish Budget Summary Sheet for completion by the Parish. We have found that this worksheet assists with summarizing programs as well as verifying overall program percentages. Following are instructions for completion: 1) Go to the worksheet on the Budget tab. In general, the blue areas will fill in automatically and white areas are either pull down selection of free space for typing. The table is currently completed with sample entries which should be overwritten or deleted to reflect your Parish Proposal. 2) Select your Parish at the top of the worksheet. By placing your cursor on the box, you will see a pull down option appear listing all of the parish names. When your parish is selected, your allocation is filled in automatically. 3) Select a box under Project Type and a pull down arrow will appear to the right. These are all of the eligible project categories as listed in the Gustav/Ike Action Plan. Select one for each proposed activity you would like to insert. 4) Under Project Description, type in the name/title of your project. 5) Under Estimated Budget, type in the budget for the project. The % of budget will fill in automatically and keep a running total of your allocation as you insert all of your activities. The Blue areas fill in automatically to help you track calculations. 6) Under Primary/Alternate, select whether this is a primary project for your parish or an alternative project for your parish from the drop down list. 7) Please complete the question on affordable rental at the bottom of the sheet. 8) Only the 5 Parishes with highest damage estimates have received a direct Affordable Rental Housing Allocation, which is reflected at the top of the sheet when that parish is selected. Only those 5 parishes (Terre, Cam, Iberville, EBR, Lafourche) should complete the worksheet on the tab labeled Affordable Rental. These funds are tracked separately from the Parish allocation. 9) Please submit a printout of this sheet(s) with your Parish Recovery Proposal and also email an electronic copy to your Parish Outreach representative.
