PURPOSE:

To describe the process for obtaining an outside vendor repair service.

POLICY:

        Under some circumstances, an outside vendor repair service shall be required.

        Prior to initiating the request with the vendor, __________________ shall obtain a
         purchase order (this includes repairs completed under service contracts).

        After repairs are completed, __________________ shall obtain proof of repair (blind
         receiver) from the repair person.

PROCEDURE:

        Obtain purchase order number for repairs which will be completed by an outside vendor or
         a manufacturer’s service department.

        Submit a request to the vendor for the repair.

        Obtain a proof of repair from the repair person which documents the work/service that was
         completed.

        Keep a copy of the service records on file.

        Forward service records to receiving location, if equipment is transferred.
